Corrective SummaryDealers will replace the hazard switch carrier. owner notification will be conducted in two phases. the first phase began april 15, 2002, with mailings to owners of 2000 and early 2001 vehicles. the second mailing phase will be to remaining 2001 and 2002 vehicles. owners who take their vehicles to an authorized dealer on an agreed upon service date and do not receive the free remedy within a reasonable time should contact chevrolet at 1-800-222-1020, gmc at 1-800-462-8782, or oldsmobile at 1-800-442-6537.

 On november 30, 2001, general motors (gm) conducted safety recall 01v-364 for the subject vehicles built between september 16, 1999 and august 28, 2001 to remedy a defective hazard switch that would cause a loss of hazard and stop lamp illumination.the hazard switch is contained within the vehicle multifunction switch.odi opened this investigation based on reports that some consumers were not notified of the original recall or that they had repairs done under 01v-364 but experienced subsequent multifunction switch failures.the multifunction switch supports the operation of cruise control, windshield wipers, exterior lighting and turn signals in addition to hazard and stop lamps. on may 21, 2008, gm notified the agency that it erroneously missed identifying 5,862 vehicles that should have been included in recall 01v-364.gm will now notify owners of these vehicles under a new nhtsa recall number 08v-231.owner notification began on june 17.the primary cause of brake lamp failure was a design change from copper to brass contact material in the hazard switch.this change was effective for vehicles built after september 16, 1999.gm changed the material back to copper for vehicles built after august 28, 2001. the remedy part for both 01v-364 and 08v-231 contains copper contacts.gm's data shows that less than 1% of vehicles repaired under recall 01v-364 required additional subsequent repair or replacement of the hazard or multifunction switch.this investigation is closed.

 Rear lights do not brighten when braking, took it to dealer. dealer says there was a recall and it was done already to my 2001 blazer, but i never had taken it in for that problem was going to charged 500 dollars just to redo it. i had bought the blazer new from a dealer.

 After finding out my 2001 chevy blazer had no brake lights the second time since new, this first went out & was replaced by avenue chevy, batavia on 11/7/2001 w/ only14,790 miles . and not imagining the problem could be the expensive entire turn signal unit again, i asked around, even guys at avenue chevy, batavia, ill. i purchased a brake switch part # 15128745 on 3/4/08 for $14.09 & replaced ny unit but this did not fix the problem. i went back to avenue chevy and on 3/11/08 they replaced part # 26100937 multifunction switch for a total of $442.79 ! ! ! *tr

 The brake lights in the back of my 01 chevy blazer go out from time to time. we got the recall info from chevy and took it to the dealer. chevy said they fix the problem before we had the car thus they are off the hook for any liability. we bought the car used with only 20,000 miles and the problem still is occurring with the car. the cost to fix this problem is close to 1000 dollars and i feel that the problem is still occurring even though the manufacturer said they fixed it. i feel that chevy should still fix this problem because it's obvious it wasn't done right on the first attempt and more importantly it's a very dangerous situation. *tr

 2001 chevrolet blazer developing various problems.***no answer required***. *mr the differential pinion shaft seal was leaking. when it rained the transmission slipped, the cigarette lighter was inoperative, the lighter socket was replaced. the heater was not blowing hot air, the service engine soon light illuminated, the brake light was inoperable, the directional switch was replaced. the front rear rotors were rotted, the rear wheel well molding was coming undone, the drivers window was leaking water. the vehicle was not shifting properly, the alternator was replaced, the tail gate glass would not lock, the driver door did not shut easily, the battery was replaced. the headlights were inoperable. the driver seat recliner was replaced, the retractor in the seat belt was reset. the rear window molding was loose. the oil cooler lines were leaking.*jb

 (1) we own a 2001 blazer. in the past our vehicle has been rear ended twice, my wife suffered a neck injury in the 1st accident. the 2nd accident just happened about a month or so ago. (2) 2-19-2005 i was following my wife home ,after having my other vehicle serviced,and noticed the stop lamps were not working. i got on the internet ,& found there was a recall for 2000--2002 blazers with the exact same problem nhtsa recall 01v364/gm. (3) i called gm at 1-800-222-1020 gave them my vin number,they said my vehicle was not part of the recall .but i should take to vehicle to a gm dealer as soon as possible ,have the multi-switch repaired,& they would reimburse me for the expenses then ,the first person i spoke with never called me back as they stated they would,i called him back twice. after no response i called ,& spoke with another person , she was going to call the location i had the multi-switch replaced,& call me back , her co-worker called me ,& told me she was in a meeting ,& i should call her back. since then i have called her back 3 times,just got her voice mail ! still waiting to hear back from her. i can not believe gm is not taking this safety problem serious!! my wife,& children could have been killed because of this defect. if our blazer is not part of a recall then gm must be trying to hide this problem!. *ak

 The 4 ways do not work and are effected by the same components as the brake lights. since the brake lights sometimes work but most of the time do not, the dealership will not replace these lights. the dealership stated that they worked today when they checked them out and said there was no recall on this particular part. my concern is my family getting rear-ended by another vehicle or me getting pulled over by a police officer and getting a ticket. this is a serious problem that could possibly affect other people as well as my passengers.*ak

 Brake lights will not work when applying brakes. bulbs and fuses are all ok, hazard flasher when flashing causes an electrical static sound under dash and when wiggling the hazard flasher switch, brake lights will work and come on. i also own a 1996 chevrolet blazer that had the same problem and was repaired at bob hook chevrolet 09/07/99 invoice no. 156312. very little chance that i own two of the same vehicles five years apart and same defect happens to both. this is a serious safety problem that needs to be recalled before accidents occur because the vehicle behind didn't see vehicle in front stop. i am not only concerned about my family being safe but for the safety of others! why would the dealer service shop tell me that the part will have to be ordered and there is a national back order on that part but to continue to drive the vehicle? i have contacted gm. and filed a complaint # c05965366 and they said; we will do the best we can to get the part shipped. please help get this defect recalled so potential accidents can be avoid. *ak

 Both the brake lights and turn signals suddenly stopped working. when i took the car into the dealer, they informed me immediately that this problem was occurring on many similar vehicles. they confirmed that the problem was caused by the failure of the multi function switch. in fact, this problem was so wide spread, the part was on national backorder and would not be avialable 10 days to two weeks. it appears that general motors does not think that thousands of cars dirving around without brake lights or turn signals is a serious problem. *ak

 I went to have my car inspected when i was informed that i had no brake lights. the station where i got the inspection told me this was not the only gm car they had seen with this problem. they told me they had seen this afew times in thelast month. i went to my dealership and they told me i needed a multi function switch. i called gm to form a complaint because there is no telling how long this had been like this. lisa at gm told me that they had failures in this switch and they were looking for a more dependable source. i couldn't even get a switch for a week because of this . driving with no brake lights could be a potentially hazardous situation. i'm reporting this so someone doesn't loose their life over these defective switches. please contact me if you need additional information. thank you, joel collins. *ak

 I was driving on a road when someone pulled up next to me and told me i didn't have brake lights. only the small top center brake light was working. i was lucky that a stranger told me, i would never have known that they were not functioning. when i called the dealership they said that there was a silent recall. they had 3 other vehicles in that day with the same problem. i feel that this is a major problem because if someone isn't paying complete attention they will not notice that a blazer has stopped until it is too late. i feel that there should be a public recall for this problem. it was a simple fix (it only took part of one day). all they had to do was put in a new turn signal lever. i think that any problem like this should always be a public recall, it is not fair that the manufacturer and dealership knows about a problem and does not tell the consumer. i guess they would have told me had i gotten into an accident. but then it may have been too late. thank you very much. janet l. werner. *ak

 Tl*the contact owns a 2001 chevrolet blazer. while driving approximately 35 mph, the contact noticed smoke coming from the steering column and the brake lights were inoperable. when the ignition was turned off, the engine would still operate. the contact had to make several attempts to turn off the vehicle. there were no warning lights illuminated on the instrument panel. the vehicle was taken to an authorized dealer and they stated that the failures were related to a defective multi-function switch. the vehicle was repaired. the contact experienced failures identical to the ones mentioned in a prior two part recall. the consumer assumed the recall was performed, however she learned that the recall was not performed completely. the consumer began having problems with the multi-function switch. the part that the consumer thought was replaced back in 2002 was never replaced. the failure and current mileages were 31,138. the consumer is seeking reibursement. updated 09/19/08. *jb

 I have a 2001 chevrolet blazer that recently had to be repaired. i feel that a recall item should cover cost of the repair. the chevrolet customer representative that i spoke to said that it is not covered by the recall. in a nutshell, the symptoms that the car had are exactly the same as those mentioned in nhtsa campaign id number : 01v364000. both rear, outside stoplights stopped working. the center stop and the turn signals worked fine. i had the car repaired at a cost of $470.29. on the invoice the item replaced is a multifunction switch? *tr

 I was in a drive through which looped around the building. i applied the brake to stop at the window and could feel the abs engage and the vehicle did not want to stop. it scared me and of course i reapplied the brake quickly, to avoid hitting the vehicle in front of me, and i was finally able to stop. this happened again a few weeks later at a large intersection at a red light. i was 7 months pregnant at the time and was pretty scared. i have heard several of these complaints and i know that the brakes on every year before this were recalled. i also had trouble with the brake lights and four-ways, which stopped working. i was driving and the four ways came on. i had to pull over and ended up holding the button down while i drove in order for them to stop flashing. shortly after the turn signals stopped working. finally, the speedometer temporarily stopped working, then shortly after that, the heat stopped working, the vehicle only blowing cold air. we replaced the thermostat but the heat still would not function. i ended up getting rid of the vehicle because of the numerous malfunctions. *jb

 I own a 2001 chevy blazer, my brake lights and hazard lights stopped working for no apparent reason. the bulbs are in working condition, the fuses are not blown. i then did an internet search and found out that the 2001 blazers were recalled for this very same problem nhtsa campaign 01v364000, so i phoned a chevy dealer and was told my vin # was not included in the recall and it was over i don't feel that i should be burdened with this problem when it is clearly defective manufacturing. *jb

 I bought my chevy blazer in may of 2001. we had a recall for the exterior lighting: brake lights: switch. we brought it back to the dealer we bought it from and they did replace it. we are having the problems with the same part once again (no brake lights). we called the dealer and they're telling us this part is not covered. if there's still a problem with it, which i understand we're not the only ones reporting this, why wouldn't this be covered? my husband has owned his gm truck for over 10 years and has never had to replace this part. why would a newer vehicle have to replace it twice within a 4 year period? please advise.*jb

 2001 blazer has defective brake lights. this is intermittent, and is related to the hazard flasher switch assembly. there is a recall on this issue for selected vins. gm will not cover this problem on my vehicle which has experienced this problem at least 2 times in the past year. *ak

 Tl*the contact owns a 2001 chevrolet blazer. all of the brake lights malfunctioned except for the window brake light. the vehicle was not inspected by a dealer. the manufacturer stated that the vin was not included in nhtsa campaign id number 01v364000 (exterior lighting:hazard flashing warning lights:switch). the current and failure mileages were 31,000.

 I was in a drive through which looped around the building. i applied the brake to stop at the window and could feel the abs engage and the vehicle did not want to stop. it scared me and of course i reapplied the brake quickly, to avoid hitting the vehicle in front of me, and i was finally able to stop. this happened again a few weeks later at a large intersection at a red light. i was 7 months pregnant at the time and was pretty scared. i have heard several of these complaints and i know that the brakes on every year before this were recalled. i also had trouble with the brake lights and four-ways, which stopped working. i was driving and the four ways came on. i had to pull over and ended up holding the button down while i drove in order for them to stop flashing. shortly after the turn signals stopped working. finally, the speedometer temporarily stopped working, then shortly after that, the heat stopped working, the vehicle only blowing cold air. we replaced the thermostat but the heat still would not function. i ended up getting rid of the vehicle because of the numerous malfunctions. *jb

 A person in a vehicle that had been behind my vehicle stopped me and informed me that i had no break lights. i called gm motors and asked them to fix the problem because i had a piece of paper stuffed in the hazard switch on the steering column so my break lights would work. i was told by gm that my vehicle did not fall under the recall by the vin number. *nm

 (1) we own a 2001 blazer. in the past our vehicle has been rear ended twice, my wife suffered a neck injury in the 1st accident. the 2nd accident just happened about a month or so ago. (2) 2-19-2005 i was following my wife home ,after having my other vehicle serviced,and noticed the stop lamps were not working. i got on the internet ,& found there was a recall for 2000--2002 blazers with the exact same problem nhtsa recall 01v364/gm. (3) i called gm at 1-800-222-1020 gave them my vin number,they said my vehicle was not part of the recall .but i should take to vehicle to a gm dealer as soon as possible ,have the multi-switch repaired,& they would reimburse me for the expenses then ,the first person i spoke with never called me back as they stated they would,i called him back twice. after no response i called ,& spoke with another person , she was going to call the location i had the multi-switch replaced,& call me back , her co-worker called me ,& told me she was in a meeting ,& i should call her back. since then i have called her back 3 times,just got her voice mail ! still waiting to hear back from her. i can not believe gm is not taking this safety problem serious!! my wife,& children could have been killed because of this defect. if our blazer is not part of a recall then gm must be trying to hide this problem!. *ak

 Both the brake lights and turn signals suddenly stopped working. when i took the car into the dealer, they informed me immediately that this problem was occurring on many similar vehicles. they confirmed that the problem was caused by the failure of the multi function switch. in fact, this problem was so wide spread, the part was on national backorder and would not be avialable 10 days to two weeks. it appears that general motors does not think that thousands of cars dirving around without brake lights or turn signals is a serious problem. *ak

 To whom it may concern,i'm very upset and dissatisfied first of all the first day i purchased the 2001 chevrolet blazer i had it not even a week and people where noticing the drivers side must have been damaged at the factory,so i brought it back and phillips chevrolet was shocked to see this also,and i am leasing this vehicle and definitely was going get blamed for something that was defected from the start.they where great the exchanged the vehicle for me and this is a lemon also i am very upset at the fact that this is 4times now that my fuel injectors have messed up and i have taken this vehicle 2 times to o'conner chevrolet where they have replaced one and cleaned them both times.so then it happens for a third time so i take it to phillips where i bought it thinking the will take beeter care of there vehicle,they cleah 3 injectors 3 times they said and reset the codes, and told me to use shell gas so i did this and now exactly a month later my engine service light came on again3/29/02.now im very upset to where i dont even want this vehicle. also the multi-function switch has been replaced.so now you tell me should i be happy i dont think so.icant afford to keep taking off from my job for this inconvience.what a joke. *ak

 I purchased my vehicle in 4/01 and seven months later i began experiencing a host of problems with my vehicle, starting with multifunction switch worn, then hood hinges worn, exhaust (muffler broken) right speaker inoperable, left front seat adjuster broken, grill assembly loose, brake pedal broken and finally transmission failure (needs replacing). i have done a bit of inquiring to other chevy blazer owners and found out that they have or had experience some if not all of the same problems that i am experiencing now. i am hereby respectfully requesting your assistance in rectifying this complaint. i don't think that after 11mths of purchasing my vehicle i should be needing my transmission replace. from passed experienced with replacing a transmission on my wife new vehicle (mazda mx3) back in 1992 it has never been the same. we had been having the same problems and more with it. we were able to actually get another vehicle and the same problem keep occuring . after that long and tiring experience with my wife's car i now that this won't be no different. i have purchased a lemon and i need to know what recourse i can take to insure that i would not be stuck with a vehicle that would be costing me more to have it constantly fixed and replace. i thank you for your help and assistance in this long and tiring ordeal. i anxiously await hearing from you. respectfully yours. *ak
