 I was driving at a speed of approximately 45 mph when a driver, who was cited for failing to yield to the right of way, entered the roadway from my left. the initial impact was my truck's front to the passenger side of the vehicle that entered the roadway. after both vehicles separated, my vehicle continued and struck a median guardrail head-on (another frontal impact) before coming to rest. my seatbelt did not lock and no airbags deployed. the highway patrol investigated the active collision scene. four days later, an independent collision reconstruction expert that i hired inspected my vehicle at my local gm dealership. the inspection revealed that: i was in fact going 45mph, my seatbelt was buckled, my seatbelt pretensioner did not activate, and no airbags deployed. gm has also been contacted by me, and they are currently investigating the issue.

 Seat belt locks up when driving on smooth roads. ddoes not allow driver to lean forward to look in blind spot of drivers side window. happens on slightley bumpy roads and on smooth roads when making turns or lane changes. happens every day and can be reproduced at any time.
