The subject vehicle (sv) population includes my 2005-2006 vehicles built between march 2004 and december 2005 manufactured with a trw supplied headlight switch assembly (hsa).the hsa was the primary focus of the investigation; earlier and later vehicles utilized different hsas.consumers complain that the headlights in the subject vehicles do not illuminate when the hsa is turned on, or that the headlights extinguish while on (subject failure).no other driving lights are affected.the front marker, tail, and instrument lamps operate.also, the high beams can be illuminated by holding the turn signal stalk in the up position.as reported by consumers, turning the hsa off and on typically corrects the failure.initially the failures occur infrequently but they become progressively more frequent with age.chrysler has indicated its intention to offer of an extended lifetime warranty on the hsa, so that consumers with this problem can have their hsa replaced at no charge when they begin experiencing the subject failure.commencing sometime in early 2013, chrysler will send letters to sv owners notifying them of the lifetime warranty.the sv headlight system design is essentially the same as that described in ea 05-009, which involved headlight anomalies in earlier model chrysler minivans; refer to it to for additional information.the subject failure is caused by friction between the plastic cam and metal surfaces of the electrical contacts in the hsa, which results in the production of plastic particles (debris).analysis shows that debris can collect on the electrical contact that controls the headlights, resulting in an open circuit and preventing the headlights from turning on.subsequent operation of the hsa may temporarily displace the debris, at which time the headlights operate correctly.lubricant was applied to the plastic cam beginning in december 2005, reducing debris production.consequently, subsequent failure levels were lower.the report counts shown above include only those that involve the subject failure.reports involving other failure modes which were identified have not been included either because they were not safety related, or if they were safety related, because there was no defect trend.the single crash event listed in the failure report summary table above involved a deer strike allegedly due to loss of headlights in february 2006.no injuries were reported in this crash.an april 2004 non-crash incident (

To date, odi has interviewed several complainants who confirmed that the headlights fully extinguish, and remain extinguished, on an intermittent basis.in most cases this occurred while the vehicle was in motion.the occurrences do not show a repeatable pattern, and the problem occurs at a low frequency, typically one or more weeks between failures.for this reason most complainants have not attempted diagnosis or repair of the problem, so the cause is unknown, and the problem continues to occur.in the cases where repairs have been attempted, which consisted mainly of headlight switch and body control module replacements, mixed success is reported.in interviews complainants advised odi that they can usually toggle the headlight switch, or turn on the high beams by actuating the column stalk (pulling it upward), when the headlights unexpectedly turn off.complainants report that toggling the switch is usually effective, and the high beams have always been available when the column is used.during the course of the interviews odi was able to obtain a few suspect headlamp switches for further evaluation.no verifiable crashes or injuries have been identified in the vehicle owner complaints (voqs).chrysler notes that its complaints also include allegations of dimming or flickering of the headlights in addition to loss of headlamps, and that their analysis of available data indicates a declining trend occurring in the field.chrysler data reports two alleged crash incidents; one where a consumers vehicle lost forward lighting and struck a deer, and one where a consumer alleged their lighting stopped working, causing them to depart the roadway and drive into a ditch.a minor injury is alleged in this second incident.both incidents occurred several years ago, and neither allegation has been confirmed by odi.odi is upgrading this investigation to an engineering analysis (ea11-010) to continue its analysis of the alleged defect and its consequences, which will include additional analysis of consumer reports, their frequency and trend, and chrysler's technical information for the headlight system and its components.additionally testing will be conducted to evaluate possible root cause(s) of the alleged defect, and will include analysis of suspect components collected from the field, and/or assessment of subject vehicles that have reportedly experienced the condition.

On the interstate i-75 in tennessee mountains at night, headlights went out while driving with family of 4. quickly attempted to turn on and off the headlight switch which turned the lights back on. my teenager had complained about problems turning the headlights on previously, which i had ignored, stating that she turned the light switch on but the headlamps did not go on but that the yellow side markers did turn on. have experienced this problem repeatedly over the last few weeks. chrysler dealer tom ahl in lima ohio was informed of the issue but would not inspect or service the vehicle without charging an inspection fee, as no recall has been issued for this problem. i declined to pay this and asked them to enter the issue into their service database. *tr

I have had several incidents in last 3 weeks where all interior and exterior lighting flicker or pulsate. over the past year i have also experienced total loss of exterior lighting but only noticed it when parking or at lower speeds without causing a hazardous situation at the time of the incidents. on each of the incidents i was able to toggle the light switch several times and was able to re-activate the lights. this was an intermittent issue separated by weeks between incidents and it hasn't occurred for a couple months. the flickering is a new issue and some on-line research to the flickering led me to this. *tr

Overhead lights in console went out one at a time (one had already been replaced within 1st yr.). i went in to buy replacement bulbs at the dealership. the design has changed and part of the plastic has to be trimmed to make it fit. they put the bulbs in and it still didn't work. they said the wiring must have melted so you need to order the entire overhead console. after hearing this i wondered why there had not been a recall - wiring burned? and, it sounded as though they were familiar with this problem, i was not the first. my husband looked at it today and he got the light to work but not with the switch. it is getting power but it won't operate the bulb. they had said part of the plastic on the bulbs melt. once again, melting bulb base not a recall issue? chrysler told me the dealership needs to troubleshoot to find the problem - with me paying $92.50/hr.! there are several instances of this happening which i found on the internet - lots of bulbs having to be replaced and since they redesigned the bulbs the do not work. *tr

Headlights will not turn on. repeated turning off & on of headlight switch (5 to 10 itmes) eventually works. however, using the turn signal lever on the column to hold the brights on does result in brights only - if you hold the lever continuously. nhtsa ea11010 from september of 2011 says defect is being investigated. how much time is needed for a diagnosis? *tr

Headlights turned off while driving at night.

Loss of headlights while traveling 70mph. high beams worked only if lever was held down, and i had to drive like this for an hour. over the past 2-3 years the headlights would intermittently flicker or turn off, but i could usually resolve the problem by toggling the headlights off and on several times. this would occur periodically, with several months in between incidents. in may and june of this year, the problem began to occur more frequently, with complete loss of headlights finally occurring. my mechanic replaced the front headlight control module 6-15-12 and the headlights operated fine until this past week- 7-11-12, when the headlights began to flicker again and went out while driving only 5mph in parking garage. toggling the on/off switch corrected the problem and headlights came on.
06/01/2012960001
The headlights will fail without warning. i was driving at night and noticed that the headlights did not turn on when the switch was activated. toggling the switch back and forth a few times, turned them on. since the first incident, this has occurred at least 2 times per week. the lights will usually turn on when switching the lights on and off, but the past two days they have not turned on at all. the parking lights and highbeams still work, but not the main driving lights. this is a serious safety problem. the dealership does not appear to know what is going on. *tr
05/07/2010
Headlights spontaneously stop projecting light. sometimes, playing with the switch seems to help. neither high beam nor low beam light positions work so long as the lighting system is malfunctioning. seems to happen in particularly humid situations. mechanics have not been able to isolate the problem at this time, but frequency of malfunction varies making it difficult to determine if their solution was effective. it is very dangerous to be driving and have the headlights go out.

My 2005 chrysler town & country has had the headlights fail several times. high beams worked by holding them in the on position, if i tried to turn them on they would go out. after flipping the switch a number of times i got the lights back on. we just lost lights again last night and were able to get them on after flipping the switch a number of times.

Went to turn headlights on and they would not turn on. had this happen once before, but i just turned the switch on and off repeatedly and they finally worked. i tried a few different things this time and nothing worked. they lights still don't turn on. this seems to be a dangerous problem! i looked for a solution online and there seems to be many others with the same problem.

06/03/200945000
All failures progressed in sequence over a period of time. door locks failed to lock and or unlock intermittantly using power controls. instrument panel lights and gages would flicker and pulse on and off, at times just off at night for different periods of time, usually minutes. headlights would flicker between brights and dims or on and off. radio lights flicker on or off. instrument panel indicators light up for minutes at a time and then shut off with apparently nothing wrong. seat belt audible alarm sounds on occasion when driving for no apparent reason. cooling fan turns on hours after parking and can only be shut off by removing fuse or draining battery. engine stalls at high speed abruptly, and turns back on. transmission disengages and then re-engages after a few seconds. occasionally the ignition switch will not provide power to the starter until after a few attempts and a few minutes.

I was traveling last night and both of my headlights went out. i am very fortunate that i did not wreck!!! the lights went off and after i toggled the headlight switch 3-4 times the lights came back on. however, about 3 minutes later, they went off again. this time it happened on a curvy road with no street lights! i toggled the switch again and they came back on. i took my van home and it has been parked ever since! i have noticed via the internet that this is a common problem with this vehicle! i hope that you all will investigate and determine that this needs to be dealt with. i had 3 of my children with me when this happened and i shutter to think of what could have happened!!!! please fix this problem. it appears that chrysler has known of this issue for a while but has failed to fix the issue. maybe you all can force them to fix it!!!

I have had the same issue as many others on this posting. my 2005 chys. twn & cntry started by the lights flickering on and off, then recently has been hard to turn on. i have to toggle on/off switch and brights to finally get them to turn on. once i get them on they often turn off by themselves and i have to pull off the road to get them back on again through the whole process. it is getting increasingly harder to operate the lights.

About may or june of this year, the headlights of our 2005 town & country van started failing to come on when the interior switch was turn on. at first, simple toggeling would get them to work though the frequency of having to do this as well as the lights failing to stay on have increased significantly in the recent months. this is my wife's primary work vehicle and she ofter returns from work when it's dark. she had a number of instances where the headligts have just gone out while she was driing. i was looking on the iternet recently to see if there were any posts as to what is causing this to happen and not many have posted an definative answer to the headlight failure. my other major issue with this 2005 town & country is the ac system. we have had this fixed over 11 times since we purchased the vehicle new in october of 2005 with the first repair coming less than a month after we purchased the van.

The headlights have suddenly gone off on several instances, with no apparent pattern. jiggling the switch works sometimes. on others, i have to drive home pulling back on the high beam switch, which is none too popular with fellow motorists. overall, this has happened 10 to 15 times. even if i turn the engine off and restart this does not necessarily solve the issue. i live in an area with roads that flood and often have no shoulders. several close calls have resulted while driving home in the rain and suddenly losing the lights. your assistance would be appreciated.

Both headlights stopped working at night. i had to hold the bright light switch on for an hour drive home in the dark. the next day they didn't work, then when i took it to the mechanic, they fiddled with the switch and got it working. but many many folks are complaining about this on-line. please issue a recall

It first started by failing to turn on after a few times of trying to turn on the lights on the dash. it then progressed by turning off while driving in the city. it worked for a few months without incident until 2 months ago. after a cold start the lights would come on but after driving it to the store about 3-4 miles away and restarting the car it has failed repeatedly. i believe this to be a serious safety issue. i've had to drive home by manually holding the high beams on to get home. there was almost an accident due to this problem.

The headlights don't come on right-a-way. it takes about 4-10 attempts before they come on. a couple of times they turned off a second or two after a successful attempt to turn them on. this has been going on for the past 2 months, but now it has become more frequent to the point where it happens every time we need to use the lights.

I have a 2005 chrysler town & country. tonight at 8:30 i turned the light switch on and started out of a parking lot when i realized that the headlights were not on. after several attemps of flipping the light switch on and off with no success, i put the vehicle in park and tried several more times and was finally able to get the headlights to turn on. 15 minutes later, while traveling down the road at 40+mph with 3 month old twin grandbabies and two older children in the vehicle, the lights turn off for at least 20 seconds or more and then return back on for even less time, when they resume to turning off again several times before making it home. i purchased this vehicle from a used dealership in september of 2010 with no idea that there were so many complaints about the headlights failing. this vehicle was purchased with the intention to safely carry my twin grandchildren and it is not acceptable that this problem has not been resolved and the dealership is not being held accountable.

03/04/2011220000
It happened several times at night when driving the headlights turned off. pulled over and tried to turn the switch on and off several times before it turned back on. this kept coming back every now and then. some times the lights will quit working for several weeks and then one day they would just come back on, and stay on for a few days before it starts all over again. replaced headlight switch and headlight bulbs, clean all the fuses in the fuse box, problem not solved. this problem compromises the safety of the occupants of this vehicle as well as the safety of other road users

I have had problems with my headlights going off while driving at night or not coming on when i turn on the switch for going on 3 years. while my van was under warranty it was taken in specifically for this reason only to be told that they could not replicate the problem. it is now out of warranty. it can go for a couple of weeks without a problem then all of a sudden the lights go out while driving down the interstate. luckily i have been able to get them to come right back on. but i have had several instances where the lights just will not come on without turning the switch off and on several times. this is really getting old fast. i bought the van for it's stow and go seating but i am beginning to wonder if all this is worth it.

The front passenger airbag light often comes on on my 2005 chrysler town and country when the steering wheel is turned. i took it to the dealer when this happened the first time, and they replaced the wiring harness for the airbag, but the problem still occurs. also, the headlamps often fail to turn on when the the switch is turned. turning the switch on and off several times usually solves the problem, but lately it seems to be getting worse.

Headlights won't turn on when headlight switch is turned on. from inside the vehicle, it appears the headlights are on, but if you look at the headlights they are not illuminated. to make sure the lights are on, the vehicle has to be started, the headlight switch turned on, and the driver or passenger must get out of the vehicle to visually check if the headlights are on. if they aren't, the headlight switch must be turned on and off several times before the lights will work. the headlight failure occurs about every other time the headlights are required. the dealership reprogrammed the bcm, but that did not help and they can't figure out the problem.

Several times over the past few years, the headlights would not turn on. also several times, the headlights would just shut off while driving. the parking lights would stay on. usually we could get the headlights to come back on after trying to turn them on and off several times via the switch. the most recent was yesterday, 20 may.

While driving down the road at night on the 15th of may, 2010, the vehicle headlights went out several times. i was eventually able to get them to come back on, but can not state how. it first happened while turning the bright lights off, but later occurred without using the brights. the main light switch also appeared not to matter. a few times, i cycled the main light switch and they came back on. most times, i would cycle the main lighting switch and the lights would not turn on. searching the internet, it appears that this is a known intermittent problem with both dodge caravans and chrysler town and country mini-vans. this happened again this morning when attempting to leave. it took several minutes to get the light to turn on. *tr

My headlights will just shut off while driving on my 2005 chrysler town & country. this happens frequently at night, so i am indoors after dark. i can turn the switch multiple times and they usually come on. and now, the state of ohio passed a law that if you use your wipers, you must have your headlights on. so when i get a ticket because they shut off without my knowledge in daylight hours, who pays my fine? i am going to get it repaired this week, but its still a dangerous situation until then! *tr

Headlamps go off intermittently. movement of headlamp switch will turn them back on, but, interior lights are dead when opening door and overhead computer is dead when lights are on. dangerous situation for the headlamps as it could cause a wreck. reading online blogs this seems to be a bad problem with the 2005 model year chrysler town and country's and dodge caravan's. does someone have to get killed first to warrant an nhtsa investigation? i did a search on investigations for my vehicle year and type and only found the air bag investigations. this is a very common issue with not only my year but other years of chrysler mini-vans. it needs attention! *tr

Headlights go off while driving. sometimes the lights come back on after a few seconds, after turning the switch on and off repeatedly or sometimes the light do not come on until after the vehicle is stopped and ignition turn off and vehicle restarted. the problem is suppose to be a multifunction switch. when you are driving 55 mph down the road, this is not safe. *tr

I have the same intermittent headlight problem as described in the below article but my chrysler town & country is a 2005. it appears the problem is not limited to 2001-2002 model years. i am afraid to drive this vehicle at night. the headlights sometimes suddenly turn off while driving (although the running lights stay on) or when starting up the vehicle the headlights won't turn on. was there ever an official recall for this? http://www.autotalkusa.com/recalls1.asp?make=chrysler date: 06/08/05 make: chrysler nhtsa investigates chrysler minivan headlight problems june 2005 the national highway traffic safety administration (nhtsa) has upgraded an investigation of 813,597 chrysler minivans including dodge caravan and grand caravan, plymouth voyager and grand voyager and chrysler town & country and voyager minivans from the 2001 and 2002 model years to check for problems with headlights, the dow jones news wires reported. the investigation is a response to consumer reports that the headlights are flickering, dimming and periodically shutting down while the vehicle is in motion. the problem has led to the minivans being towed in some cases. the company has received 675 complaints and more than 35,000 warranty claims related to the alleged defect, nhtsa said. nhtsa investigations can lead to vehicle recalls. *jb

The headlights on our new 2005 chrysler town and country minivan turn off during operation. we've taken it to the garage (it's still under warranty) and the mechanic advises that he has seen this same problem on several vehicles. he was unable to locate the cause of the problem. this is an extremely dangerous condition at night, as the driving lights cut out without warning and do not reactivate by turning the light switch off and on. *jb

Headlights will not turn on but dash lights will. head lights will go off while driving. this is an intermittent problem that requires repeated turning on and off of the switch to get them to come back on. the dealer said the swith can be replaced for $320.00, and is not covered under warranty. why should i replace a switch with another that will have the same issue? dealer shrugged shoulders. *tr

While driving at night the headlights turn off. this is the second time in about a year that the same problem has occurred. the last time they replaced the headlight switch under warranty. this time isn't covered? once the lights go out you have to constantly turn the switch back and forth until they come on. i talked with chrysler customer reps today and they say it's an isolated or unheard of problem. however, according to this site, it seems to be more often than chrysler will admit! *nm
