Defect SummaryBoulevard is recalling 3 vans equipped with ricon platform style wheelchair lifts. these lifts fail to comply with the requirements of federal motor vehicle safety standard no. 404, platform lift installations on motor vehicles. the threshold warning system may not detect the presence of a wheelchair or mobility aid user in a certain spot within the defined threshold area.
Consequence SummaryThe user of the lift could be injured should the lift move unintentionally.
Corrective SummaryBoulevard is working with ricon to correct the wheelchair lift free of charge (please see 07e095). owners may contact ricon at 1-818-267-3000.

Defect SummaryComm-trans is recalling certain model year 2007-2009 ada conversion vans built on ford e-150, e-250, and e-350 chassis between december 7, 2007 and december 23, 2009 and fitted with certain ricon s series and k series l-model wheelchair lifts. due to a problem in the software that controls the lift's up function, should the up button be released before the floor level limit switch is activated, the lift may continue its upward travel and may overrun the floor level cut off point.
Consequence SummaryWhen the platform overruns the vehicle floor level and stops at an angle from 15-20 degrees, it is possible for a person in a wheelchair to tip over and for someone standing on the platform to fall, possibly resulting in injuries.
Corrective SummaryComm-trans will notify owners and the repairs will be performed by authorized ricon dealers free of charge. a software solution for this safety defect is under investigation. until the software solution is available, ricon will provide owners with a bulletin outlining proper wheelchair lift operation and will ship owners a dvd-based training aid to promote proper lift operation at no cost. the safety recall is expected to begin during june 2010. owners may contact ricon at 1-818-267-3000 or comm-trans at 1-901-726-9394.
NotesThis recall only pertains to vehicles altered by comm-trans and has no relation to any vehicles manufactured by ford motor company.owners may also contact the national highway traffic safety administration's vehicle safety hotline at 1-888-327-4236 (tty 1-800-424-9153), or go to http://www.safercar.gov .

Defect SummaryComm-trans is recalling certain model year 2007-2009 ada conversion vans built on ford e-150, e-250, and e-350 chassis between december 7, 2007 and december 23, 2009 and fitted with ricon public use lifts. the restraint belt on these lifts was designed to prevent the lift from moving, unless the tongue of the belt is fully engaged and latched in the buckle. ricon has discovered, however, that it is possible for the operator to insert the belt tongue into the buckle without fully engaging the latching mechanism. in this case, the restraint belt would appear to be latched, and the operator could run the lift. if the restraint belt tongue is not fully engaged into the restraint belt buckle, and latched, the lift may operate with an occupant on the platform.
Consequence SummaryThe user of the lift could be injured should the lift move without the restraint belt properly buckled.
Corrective SummaryComm-trans is working with ricon and ricon will be performing the remedy for this campaign. please see ricon's defect report 09e-060. ricon will provide customers with a warning decal and a dvd-based training aid. the safety recall is expected to begin during june 2010. owners may contact ricon at 1-818-267-3000 or comm-trans at 1-901-726-9394.

EA10003FORD MOTOR COMPANY04/13/201003/05/2012HVAC Switch Failures
This investigation was opened based on one complaint and reports from four transportation fleets alleging 182 blower motor control switch failures.odi was concerned that these melting and burning switches could lead to an open flame that could spread throughout the vehicle.after a comprehensive review of all data related to overheating blower motor control switches in the subject vehicles, odi did not identify the existence of a safety defect trend.odi reviewed 1,036 complaints, claims, and field reports of failed blower motor control switches and determined that only two fires were likely to have been caused by a failing subject blower motor control switch.this represents a failure rate of 0.2 r/100k on a population of 1,076,975 vehicles that have been in service for up to 14 years.odi has found that when the subject switches do fail and begin to melt, they are constructed in a manner that typically prevents the failing switch from igniting and developing into an open flame.ford motor company has agreed to take two actions to further reduce the likelihood an overheated blower motor switch in the subject vehicles.ford informed odi that it has implemented a series of design changes to the subject switch that will help prevent it from developing internal high resistance short circuits.ford has also informed odi that it would release a technical service bulletin (tsb-11-11-21) instructing subject vehicle owners to replace the electrical connector at the same time a failed switch is replaced.in light of the low rate of failure resulting in open flame and fire spread on the subject vehicles, and ford's actions to reduce the likelihood of a switch failure through switch design changes and a tsb, further use of agency resources does not appear to be warranted.the closing of this investigation does not constitute a finding by nhtsa that no safety-related defect exists.the agency reserves the right to take further action if warranted by the circumstances.the odi complaints cited above can be reviewed at www-odi.nhtsa.dot.gov/complaints under the following (odi) numbers: 10427875, 10114393, 10257890for additional information, see the attached closing report.

Odi has received 1 complaint and reports from 4 transportation fleets alleging 182 switch failures.each fleet operates mini buses built on a ford e350 or e450 cutaway chassis.the oem hvac blower switches have melted, smoked, burned or experienced open flame and become inoperable on vehicles in each fleet.some drivers have reported a burning smell or smoke coming from the dash.one fleet reported a vehicle fire.no injuries were sustained.however, the bus was carrying special needs children when the driver noticed smoke coming from the dash.the driver was able to evacuate the children prior to the vehicle becoming engulfed in flames.the 55 fires referred to in the manufacturers column is based on the early warning reporting definition of fire, which is as follows: fire means combustion or burning of material in or from a vehicle as evidenced by flame. the term also includes, but is not limited to, thermal events and fire-related phenomena such as smoke and melt, but does not include events and phenomena associated with a normally functioning vehicle such as combustion of fuel within an engine or exhaust from an engine.this engineering analysis (ea) has been opened to determine if the blower motor switch failures create a situation where a vehicle fire is possible and presents a safety risk to the driver and other passengers.

When the engine on the 2008 e250 ford cargo van was started and the truck put into the drive gear, the engine raced at a high rpm rate sufficient to spin the rear wheels, lunge forward under what felt like full power, slammed through a fence and down into a ravine. the van was a rental from enterprise leasing co. the passenger's right ring finger was severed in the crash. no air bags deployed and apparently never installed in the 2008 van. the accelerator appeared to be stuck in the out-of-control run-away. *tr
