Defect SummaryCertain anzo combination headlamp assemblies sold as replacement lamps for use on certain passenger vehicles. some combination lamps are not equipped with amber side reflectors which fail to conform to federal motor vehicle safety standard no. 108, lamps, reflective devices, and associated equipment.
Consequence SummaryWithout the amber reflectors, the vehicle will be poorly illuminated, possibly resulting in a vehicle crash.
Corrective SummaryAnzo will notify owners and replace the headlamps free of charge. the recall began may 9, 2006. owners may contact anzo usa at 909-468-3688.

 Tl* the contact owns a 2005 ford f150. the contact stated that the headlights became inoperable while driving 55 mph. the contact stated that he was driving with the low beams activated when they failed and never re-illuminated. the dealer was notified who advised that wither the multi-switch or the module under the dashboard would need replacing. there were no recalls for the failure. the manufacturer was not notified. the failure mileage was 164,000.

 Fog lamps on 2005 ford, f150 trucks were leaking through pressure relief hole. this caused water to drip into halogen bulb, and then bulb exploded. due to this problem many states will not pass this in state safety inspections. *ak
