Defect SummaryFord is recalling 605,684 my 2005-2006 f-150 and lincoln mark lt vehicles equipped with 5.4l 3-valve engines. the brake booster vacuum supply hose attached to the intake manifold fitting may swell over time and lose retention force. this condition could allow the hose to become detached from the fitting during an intake manifold backfire. should the hose detach from the intake manifold, several brake applications with power assist will be available before the vacuum reserve is depleted.
Consequence SummaryOnce the brake booster vacuum reserve is depleted, the driver will have foundation brake function, but with increased brake pedal effort. reduced vehicle braking could lead to a crash.
Corrective SummaryDealers will replace a rubber hose on the brake booster vacuum supply tube assembly. the recall began on june 27, 2008. owners may contact ford at 1-800-392-3673.

By letter dated may 5, 2008, ford motor company (ford) notified the agency that it will conduct a safety recall (nhtsa recall number 08v-208) to address a manufacturing defect in approximately 605,684 model year (my) 2005 and 2006 ford f-150 and lincoln mark lt vehicles originally equipped with the 5.4l displacement, 3-valve per cylinder, v-8 engine.the recalled vehicles were built from july 18, 2004 through may 31, 2006 at the dearborn truck plant; from may 4, 2004 through may 31, 2006 at the kansas city assembly plant; and from april 19, 2004 through may 31, 2006 at the norfolk truck plant.the vehicles were manufactured with the same defective brake booster vacuum supply hose assembly unique to the 5.4l v-8 engine.ford f-150 vehicles equipped with optionally available 4.2l v6 and 4.6l v-8 engines are not included in this recall action because they utilize a different, one-piece brake booster vacuum supply hose assembly that does not exhibit an apparent defect trend. the recalled vehicles utilize a two-piece brake booster vacuum supply hose assembly comprised of a standpipe assembly and vacuum hose.the standpipe assembly attaches to a fitting on the engine air intake manifold utilizing a flexible rubber hose and plastic elbow.the vacuum hose attaches to the opposite end of the standpipe assembly and connects to a check valve fitting at the brake booster assembly.according to ford, the portion of the brake booster vacuum supply hose assembly attached to the intake manifold may swell over time and lose retention force, which may allow the hose to become detached from the intake manifold.should the brake booster vacuum supply hose assembly be disrupted, application of the brake pedal may deplete the vacuum reserve and consequently result in loss of brake power-assist. to correct the safety defect, ford will install an improved design brake booster vacuum supply hose assembly that utilizes a hose clamp at the intake manifold attachment. this remedy will be provided free of charge for all recalled vehicles.later production ford f-150 and lincoln mark lt vehicles built after may 2006 were manufactured with a hose clamp at the brake booster vacuum supply hose/intake manifold attachment and are not affected. to date, the office of defects investigation (odi) is aware of 324 non-duplicative complaints and 14 non-injury crashes related to the alleged defect in the subject vehicles.in addition, odi is aware of 4,465 subject vehicles that experienced one or more warranty claims to repair or replace the subject brake booster vacuum supply hose assembly. the action taken by ford is sufficient to resolve the issues raised by this investigation.accordingly, this investigation is closed.

The brake lights stopped working and the speed control brake pedal disconnect stopped working. speed control disconnect was achieved by using the steering wheel control but if the stopping situation had been an emergency one, an accident could have resulted as there was about a 1.5 second delay for me to figure out what was going on and correct the situation. while travelling at 70mph on i-10 with the speed control engaged, i went to exit the highway to get fuel and while depressing the brake pedal to reduce speed, the speed control began attempting to resume the original 70mph speed by revving the engine. later i determined that the alternator on the vehicle had failed and while the battery had enough voltage to keep the engine running and the voltage gauge had not moved yet, the battery did not have a full 12.5v output. when the brake pedal sensor does not have a full charging voltage input, it seems to stop working. therefore, the speed control disonnect failed and the brake lights failed to activate even though the motor and other components continued functioning normally. it seems that the brake pedal sensor is very voltage dependent (which was not the case on other vehicles i have owned). i notified ford of the situation after the incident occurred and have received nothing in reponse. i got rid of the vehicle and purchased a different one as i believe it is not safe and could result in traffic tickets/accidents (no brake lights) and speed control disconnect failures (accidents) if there are voltage fluctuations in the electrical system.

In a three car accident. the 1st car turned on their signal at the same time attempted to turn into their driveway, the 2nd car hit the 1st car from the rear, as my f-150 approached the 2nd vehicle, i applied the brakes to stop before colliding into 2nd vehicle and the brakes became soft and did not slow the f-150, the f-150 was traveling at well below the posted speed limit, air-bags did not deploy, there fore hitting the 2nd vehicle in the rear causing almost no damage to 2nd vehicle and causing an almost totaled front end to the f-150 with close to $13000 in damage to f-150. no major injuries were reported, only minor back pain to both persons in f-150 were concurred.

The following incident has been filed with the ford motor company. i want to be clear that this incident was not the result of a floor mat or driver error. at no time did the driver have her foot on the accelerator nor was there an object on, around, under, or over the accelerator. incident: on 18 february 2010 at approximately 1230 pm my wife, mother in-law, father in-law and infant daughter were proceeding northbound on water tower rd. located in bentonville, ar. there were zero distractions inside the vehicle and my wife was fully alert with no physical impairments. road conditions were dry and weather was sunny with great visibility. as they approached the intersection of water tower rd. and battlefield blvd. at approximately 5-10 miles per hour, my wife began breaking. after placing her right foot on the brake pedal, she immediately noticed/heard/felt/experienced increased acceleration and decreased breaking power during her attempts to stop. she immediately increased pressure on the brake pedal ensuring her right foot was not on the accelerator. the engine continued its efforts to increase speed while my wife fought to bring the vehicle to a stop. they proceeded into and through the vacant intersection at approximately 5-10 miles per hour and fortunately without incident. my wife

I have not had a failure at this time. the problem is the brakes on my 2005 f-150. the pedal has always been mushy and engages the brakes at different levels when applied, i never where the braking will begin. some times if i am parking a trailer where the brakes are used a lot the pedal will be almost to the floor. i did almost back into a car when parking because the brakes didn't engage until pedal was quite low. *nm

I have experienced brake failures on at least 5 occasions. it has occurred at slow speed when approaching a stop at an intersection and when behind other vehicles in traffic. the brakes appear to be engaging the anti-lock braking system and start to pulse but fail to stop. all of the incidents have occurred when the pavement was completely dry and the road surfaces were clean. all incidents have been narrow misses in traffic collisions. after each incident the brakes appear to return to normal function and only once have i been able to duplicate the failure which was in a parking lot shortly after the very first incident. during each incident i initially apply and hold the brake until i realize complete failure and then release and re-apply. except for yesterday (01/01/2006) the brakes engaged on the second attempt. yesterday the vehicle came to a stop on the third attempt but only because the initial speed was so slow the vehicle just ran out of forward momentum and the minimal braking held the vehicle. i backed out of the intersection, braked, tested stopping and the brakes appeared fine at that time. i plan to take the vehicle back to the dealership tomorrow. the dealership has seen the vehicle for this problem in addition to a charging light problem and have failed to fix either problem. the dealership stated that they were unable to find a braking problem and were unable to re-create the braking malfunction. their resolution was to bleed the braking system and return the vehicle to service. *nm

At 20,000 miles i took my 2005 f150 supercrew xlt 4x4 into firestone, to have the brakes checked. the technician informed me that there appeared to be nothing wrong with the pads, as i still had about 65-70% left on them. however there was an issue with the rotors. he stated they were warped. knowing this is not normal, i asked if this was a common thing on the new model fords. and he stated that they have had a lot of them in for the exact same thing, warped rotors. i told him to put the truck back together and would take to ford. he stated that the reason they were warping, was because of a design defect which did not allow them to get enough air to cool, and if brakes were applied hard, then caused them to warp. i took the truck to ford dealership ( jenkins and wynne in clarksville, tn.) informed the service writer. as i was speaking to him, a tech from the shop walked in, and said there was a big problem with them. they took my truck into the shop, and after a day informed me that my truck had the new and improved brakes already installed. i asked how they could be new and improved if doing the same as the old style. they stated they would take care of me. when i inquired as to the how? the service writer stated we will machine them. i told him that just means that they were going to grind the rotors flat, and that it wasn't fixing the issue. i would just have to have a complete total brake replacement at 40,000 miles. he commented that there was nothing they could do. i asked if this was normal for trucks to be coming in with warped rotors at 20,000 miles, and he just shrugged his shoulders. i asked him, what are we as truck owners supposed to do, replace the rotors every 30-40,000 miles. again he just shrugged and said there was nothing he could do. after they ground the rotors, and put the truck back together, less than 500 miles they started doing the same thing, and i almost had an accident. they will pulse and the vehicle will lurch *jb

At 15,000 and 30,000 mile scheduled maintenances a brake hose inspected should be done. in our case it was completed and according to the dealership there were no found problems. however, when we were driving around a curve and there was a teenager on a four wheeler on our side of the road and we braked to avoid the child ...we had no brakes on our 2005 f-150 supercrew. we were able to avoid injuries with our family inside the vehicle and the child we almost hit due to the brake failure. upon towing the vehicle in they said that the brake hose into the intake was worn and blew off when we depressed the brake fast. i asked why this wasn't checked and they said because it was too hard to get to for a physical inspection during maintenance. the service tech also said he has seen this with several f150's. i see this as being a serious danger for these brake hoses to blow off and not be where they can be checked. we are paying for maintenance that is not being done not to mention peoples lives are at stake. *tr

The accident happened december 19, 2005: cause: brake failure. we were stopped at a red light and i was holding pressure on the brake pedal when it collapsed to the floor and rolled in to the car in front of us. at that time there was only 2000 miles on the pick-up. i wasn't sure what happened until we were at another stop light and it happened again, but this time i pumped the brake pedal until i had pressure. we returned home in march and i went to the dealer whom we purchased it from (valu ford at elbow lake, minnesota) and told them of the problem. he tried it and said he could not find a problem. i got into the pick-up and showed him how the master cylinder collapses when pressure is held on it. he tried another pick-up same year and said that's the way its supposed to be??? i went to another ford dealer in alexandria (juettner motors) and told him what happened and what i wanted done and they told me my insurance company should taken care of the problem and didn't even check the brakes. i have taken the pick-up to 5 different independent garages and they have all said the master cylinder should not collapse when pressure is held on the pedal. i went back to the dealer and ask if they could have a company tech explain to me why the master cylinder collapses when pressure is held on the pedal. i have heard nothing from them. i feel very strongly that ford has a problem with their master cylinder and should be fixed. i complained to ford again and ask them to have some one explain to me why the brakes should collapse when pressure is held on them. they sent out an independent source and he had the pick-up for about three hours and sent a report back to ford saying that the electric brakes for the camper were the probable cause?? there is no hydraulics involved with the electric brakes. nothing has been done to fix the problem.. *jb

While trying to stop my ford f-150 truck, i was actually hitting the accelerator at the same time as i was pushing the brake pedal. i could not tell that it was happening until it was too late. as i pshed on the brake the truck did not seem to slow down nearly as fast as it should have, so i pushed harder on the brake and that did not seem to work properly, then i realized the right edge of my foot was actually depressing the accelerator, but you could not really feel it since it has very little resistance. when i realized what was happening, i tried to lift my foot off the accelrator, but to do so i had to release the brake a bit. when i did that, the vehicle lurched forward toward the car in front of me and hit it. it happens because as you are hitting the accelerator while trying to brake, the engine revs up to high rpm, as you try to get your foot off the accelerator, and release the brake a bit, the car thinks you want to go fast and takes off because of the high rpm. this happened to me about two or three weeks ago, but it just seemed like an oddity at the time so i did not really think it was a big deal. then it happened to me again yesterday when i needed to stop quickly because everyone had slammed on the brakes in front of me, so when i went to hit my brakes, i inadvertantly was hitting the accelerator at the same time again, only this time, it caused an accident as i plowed into the car in front of me. i had plenty of room in front of me, at least 10 to 15 feet or more, and we were only traveling between 5 to 10 miles per hour. i wonder how many other drivers have had this occur to them. i have been driving vehicles for thirty years and have never had this happen to me until i drove this truck, and it has now happened at least 2 times in the last month. i am driving this truck since august this year. *nm

Tl*the contact owns a 2005 ford f-150. the contact attempted to stop the vehicle, but was unsuccessful. on may 18, 2008, while approaching a stop sign, the contact depressed the brake pedal and the speed reduced to 30 mph; however, the vehicle would not stop and continued through a four lane highway. she was finally able to stop the vehicle. the dealer stated that the power brake booster vacuum supply hose was falling off, and they removed the intake and glued it back into position. the dealer further stated that nhtsa campaign id number 08v208000 (service brakes, hydraulic:power assist:vacuum:hoses, lines/piping, and fittings) did not apply to her vin. the current and failure mileages were 133,077. updated 06/09/2008. *lj

Normal braking capability was suddenly and completely lost without warning when vacuum assist hose separated from fitting on the intake manifold. vacuum hose separated apparently due to some slight back-pressure that occurred when engine was started. original design did not include a clamp on the hose. correction of the problem required removal of the intake manifold to gain access to the vacuum fitting; the hose was replaced and clamped. mileage was 76,442 and extended warranty did not cover and it cost me $782. it is very disconcerting to learn that normal braking capability can be completely lost in this way without any warning whatsoever. i believe the ford motor company must be held fully responsible to correct what is clearly a design deficiency that has high potential to cause a serious accident resulting in personal injury or even death. i also feel that the ford motor company should reimburse me for cost of repair since the dealership told me that it knew of the problem and was informed to modify the vacuum hose assembly because they knew that the original composition was wrong. *tr

My vehicle stalled when trying to start. when it did start, i pressed the brakes and they were hard. i put my car in drive and tried to apply my brakes and the brakes didn't seem to work. i literally had to put 2 feet on the brakes and press down with all of my weight just to stop my truck. i brought it to the dealership and they said it was my vacuum line on my brakes that came off. it is going to cost me $375. thankfully i wasn't driving fast when my brakes gave out. *tr

My daughter was operating my 2005 model f150 when the brakes went out, she was able to swerve off of the road to avoid an accident until the truck came to rest. no physical damage was incurred to her or the vehicle. upon investigation, the brake booster had lost vacuum (power assist) due to a vacuum hose that had fallen loose from the rear of the intake manifold. 600.00 repair to re-attach vacuum hose and clamp never installed at factory. *tr

As i approached a stop sign brakes failed to brake properly. the brakes were tight and no amount of pressure would stop the truck. i put it in n and it eventually rolled to a stop with the help of gravity and the incline leading up to the intersection. i had my 2005, f150 towed to the nearest ford dealership. the faulty hose was replaced with a 'reinforced one. which leaves me to believe ford has had trouble with this part prior to this incident. why else would they replace the faulty part with this new, so-called reinforced one. the old part is not available because i was told the dealership had to send it to ford motor co. i wonder how many accidents, collisions, near misses, and deaths occurred because of brake failures that went unreported. had this failure occurred in march, just north of tailgate , all on route 77, when the tractor trailer truck pulled in front of me, i 'm sure the accident would have become another statistic reported as an unfortunate fatality caused by a careless truck driver. no one would have thought to investigate the brakes on my truck.*ak

Tl* - the contact called regarding the 2005 ford f150. the contact stated that when attempting to put the vehicle in reverse the brakes failed. the contact pulled over and turned the vehicle off. there were no warning indicators. the contact had the vehicle towed to a ford dealer. who diagnosed the failure as being a result of a lack of vacuum in brake booster. the ford dealer found that the intake vacuum connection failed because the hose had become soft. the failure mileage was 64, 277. the dealer had to remove the throttle and fuel rail to access and replace the hose. the current mileage was 66000.*ak updated 04/05/07*jb
