By letter dated may 5, 2008, ford motor company (ford) notified the agency that it will conduct a safety recall (nhtsa recall number 08v-208) to address a manufacturing defect in approximately 605,684 model year (my) 2005 and 2006 ford f-150 and lincoln mark lt vehicles originally equipped with the 5.4l displacement, 3-valve per cylinder, v-8 engine.the recalled vehicles were built from july 18, 2004 through may 31, 2006 at the dearborn truck plant; from may 4, 2004 through may 31, 2006 at the kansas city assembly plant; and from april 19, 2004 through may 31, 2006 at the norfolk truck plant.the vehicles were manufactured with the same defective brake booster vacuum supply hose assembly unique to the 5.4l v-8 engine.ford f-150 vehicles equipped with optionally available 4.2l v6 and 4.6l v-8 engines are not included in this recall action because they utilize a different, one-piece brake booster vacuum supply hose assembly that does not exhibit an apparent defect trend. the recalled vehicles utilize a two-piece brake booster vacuum supply hose assembly comprised of a standpipe assembly and vacuum hose.the standpipe assembly attaches to a fitting on the engine air intake manifold utilizing a flexible rubber hose and plastic elbow.the vacuum hose attaches to the opposite end of the standpipe assembly and connects to a check valve fitting at the brake booster assembly.according to ford, the portion of the brake booster vacuum supply hose assembly attached to the intake manifold may swell over time and lose retention force, which may allow the hose to become detached from the intake manifold.should the brake booster vacuum supply hose assembly be disrupted, application of the brake pedal may deplete the vacuum reserve and consequently result in loss of brake power-assist. to correct the safety defect, ford will install an improved design brake booster vacuum supply hose assembly that utilizes a hose clamp at the intake manifold attachment. this remedy will be provided free of charge for all recalled vehicles.later production ford f-150 and lincoln mark lt vehicles built after may 2006 were manufactured with a hose clamp at the brake booster vacuum supply hose/intake manifold attachment and are not affected. to date, the office of defects investigation (odi) is aware of 324 non-duplicative complaints and 14 non-injury crashes related to the alleged defect in the subject vehicles.in addition, odi is aware of 4,465 subject vehicles that experienced one or more warranty claims to repair or replace the subject brake booster vacuum supply hose assembly. the action taken by ford is sufficient to resolve the issues raised by this investigation.accordingly, this investigation is closed.

I was in an accident due to brake failure. i was traveling at approximately 25 mph and approaching a stop light in moderate traffic. i had 80 ft to the vehicle in front of me and when i depressed my brake pedal it went 3/4 of the way to the floor and was vibrating as anti lock was engaged. i rapidly depressed the pedal again and the same occurred. on the third attempt the brakes finally engaged but was to late to stop the vehicle as i was only 2 feet from the car in front of me resulting in a crash. my speed was reduced to 8-10mph upon impact. the smaller car in front of me was knocked into a third car stopped at the light. damage occurred to the truck and the smaller car and very minor damage to the 3rd vehicle. the weather was sunny and the streets were completely dry. i could not get my vehicle stopped even with no load other than my passenger who is a witness to the incident. no injuries occurred in any of the three vehicles and no airbags were deployed. i am scared to death as i have owned the truck for 2 years and never had this happen in my life. the brakes worked perfectly immediately following the incident and the truck was taken to the ford dealer where purchased new and all service records are at this dealer. they thoroughly inspected the entire system and found no issues. system check on all sensors, lines, rotors and pads were ok. perfect working order ? *tr

I own a 2005 f150 with a 5.4 engine. the truck is 3 years old with 49,000 miles. the brakes failed without any notice or warning. the dealer says that the vacuum hose that leads from the back of the engine to the brake booster was worn and needed replacing. the service manager at the dealership said there were no recalls on the issue and i would have to pay for the worn part and labor. after doing a little research i contacted another dealership and they said they have seen the problem before and that there were no recalls on the issue. i have a tough time understanding how this type of brake failure can occur on such a new vehicle. i have looked over everything that ford gave me when i purchased the vehicle from them. nothing says that this part should be replaced at any mileage. they concern me with fluid changes, tire wear, etc. nothing about a hose that could put my life and the lives of my family and others in danger. the part that failed is located behind the engine and the only way to see if the part was worn is to take the top of the motor off. so i can assure you this part is never inspected until the brakes fail. the dealer did fix the truck at my expense stating they could do nothing. they said they would also report the issue to ford. i also called the ford motor company customer relationship center and they agreed with the dealer . no recalls on this issue. i need to see this issue is put under the microscope. someone will be killed or seriously injured if nothing is done about this manufacturing issue. this part shouldn't fail under normal driving conditions and if does there needs to be time spent notifying the public about the issue. not sweeping it under the floor mat. *tr

Total brake failure, 2005 f-150 supercrew, king ranch 4x4 occurred in traffic on 11 nov 2005, at a speed 34 mph. when i pressed the brake pedal, there were no brakes. my would have wrecked the vehicle, had she been driving, she was hysterical when the brakes failed. i used the emergency brake and down shifted to slow the vehicle. i didn't hit the vehicle in front of me, i managed to get the vehicle to the side road parking lot. there was absolutely no indication prior to this failure. it appeared i had lost the vacuum booster assist on my vehicle. the vehicle was towed to beale pkwy lincoln mercury for repair as it is under factory warranty. the service advisor called and told me my vehicle was ready late on the 12th. i went to the dealership to pickup my vehicle and talked with the mechanic that repaired it. i asked him what he did and he told me he reconnected a vacuum hose to the back of the manifold and stated it had blown off. i ask how did you secure it. he told me by pushing it back onto the port it came off of. i ask him why not install a clamp to keep this from recurring. he stated he would have to remove the manifold to accomplish a clamp installation. beale lincoln mercury repaired my vehicle the best they could under the circumstances. i feel the mechanic should have installed something to secure the hose so this doesn't happen again, but he stated he didn't think ford would pay for removing the manifold to install a clamp. i think ford would much rather pay for a manifold r & r instead of a law suit from an accident, especially one with injuries or deaths due to a faulty design that renders the brakes useless. this has occurred on many other f-150's, some with accidents involved. check the internet for f-150 brake failures. ford needs to address the brake failure on the f-150's with 5.4l engines before there is more damage or serious injuries. this should be an immediate recall by ford. i am now uneasy driving this vehicle. *tr
