| While driving 55 mph, the driver applied the brakes and without warning the vehicle accelerated. the driver applied the brakes but failed. this caused the driver to collide with the vehicle in front. upon, impact both frontal air bags failed to deploy. the driver sustained major injuries and was transported to the hospital by ambulance. the vehicle was towed to the dealer for inspection. the cause has not been determined at this time. please provide further details. *nm | |||

| While parking, the vehicle's rubber cover on the brake pedal slipped off the pedal, causing the consumer's foot to slide off the pedal and hit against the accelerator pedal, which in turn, caused the vehicle to lurch forward and hit a concrete wall. the dealer was notified. *ak the air bag did not deploy. the consumer stated that a bent plate across the bottom of the pedal lever was the cause of the failure and accident. *tc *sc | |||
