Defect Summary | Vehicle description: mini vans. the bolts used to attach the strut to the body and the rear liftgate can accumulate fatigue damage if three factors combine. if the bolt is loose, the gas strut is over-pressurized, and the frequency of opening and closing cycles is higher than for the average customer, the bolt can fail. | |||||||||||||||
Consequence Summary | Personal injury could occur if the bolts break allowing the liftgate to fall suddenly. | |||||||||||||||
Corrective Summary | Dealers will inspect for loose fasteners, if the fasteners are loose, the strut (and integral bolts) will be replaced. if the bolt shows evidence of torque, the bolt will be removed, larger washers will replace the existing washer and the bolt will be reinstalled and hand tightened to the target torqued. | |||||||||||||||

Liftgate latches in the subject vehicles have failed in low and moderate speed impacts resulting in occupant ejections, injuries, and fatalities.there are two failure modes associated with latch failure: fork bolt-detent lever bypass, and inertial unlatching.nhtsa analyses of chrysler and peer minivans took the form of static and dynamic crash testing, engineering evaluations, complaint and field analyses, and real world data analyses.a technical report discussing these analyses can be found in the engineering analysis technical report in the public viewing file.chrysler has advised nhtsa that it will conduct a campaign to replace the latches on all the subject vehicles.the chrysler campaign will consist of notification letters to all owners; free replacement of all latches with stronger, safer latches regardless of the age of the minivans; national advertising to promote customer awareness of the campaign; quarterly reports regarding campaign completion rates; as well as additional measures to ensure a high completion rate, above and beyond the actions that normally would be taken in a formal safety recall.these include follow up written and telephonic notifications to owners who do not bring their vehicles in for corrective action.nhtsa firmly believes that the crash performance of the latches on these minivans raises serious safety concerns, and that the campaign addresses these concerns.the chrysler corrective action will increase the strength of the liftgate latches and decrease the potential for inertial unlatching, thereby reducing the risk of crash-related openings of the liftgate and possible occupant ejection through the liftgate opening.owners of chrysler minivans will get a stronger, safer latch free of charge at the earliest possible time, without the need for further technical investigation and complex and time-consuming litigation with an uncertain outcome.based on the above, odi is closing this investigation. |

2 items of concern. the airconditioning unit has failed. we own a blue grand voyager minivan and the paint is peeling from the roof and engine hood of the car. we have noticed many of the same color and year trucks, vans and cars with failing paint. a recall should happen. paint should not start peeling after 4 years of a cars life.*ak | |||

Problem #1 - rain water entering vehicle. we live in seattle, it rains 50% of the year. everytime it rains, for the last 3-4 years, water enters the vehicle. we've taken the vehicle to a factory authorized water leak detection company, after 1-2 years and about $500 in bills, they gave up. they found a new leak everytime we brought the car in. as of last thursday, 2/24/2000 , they finally gave up, said they could not justify finding more leaks!! in all they found 6 leaks, none of which were failed seals or otherwise parts that would wear out over time. all found leaks were body welds and other factory defects. we've also taken the car to glass installation stores, etc. nobody, so far can detect where all the leaks are coming from. water leak techniques said that all of the leaks they found were factory defects. what's our next step? we just want a dry car. our 2nd & 3rd problem may be related, and have been occuring almost since the car was new. basically, the car will stall while driving slowly. like when starting out at a traffic light or driving through a parking lot. the next problem, the transmission shifts roughly, usually when the engine is warm, sometimes the transmission has trouble shifting into the next gear, almost like the brain doesn't tell it to shift. we like our mini-van, we only have about 45,000 original miles on it. we have spent hundreds of dollars trying to remedy the 3 problems, can you help? this vehicle has never been in an accident. the vehicle has had regular maintenance. iread over the recall items for other 1994 chrysler models, one recall sounds similar nhtsa campaign id# 94vo24000. *ak | |||

While driving 55 mph vehicle stalled, dealer replaced a leaking fuel line. vehicle has stalled since. dealer cannot remedy.*ak a/c is cold but not blowing through the vents, only blowing through the defrost vents, also a/c was blowing out hot air, blower motor stopped working for a short period of time, due to a dead battery,fuel gauge drops to empty even though there is plenty of gas left, tires were replaced sue to excessive wear,right door electrical lock is inoperative.*jb | |||

Fatigue cracking of the driver's side door hinge pillar around the hinge. the crack has progressed roughly 50% of the way around the hinge and will lead to complete separation of the hinge from the pillar without repair. this car has had no accidents. the incident date is the approximate date the crack was first detected. *jb | |||

The rear sliding door came off the track when trying to close it. it appears that the rubber rollers deterioated and allowed the rear sliding mechanism to come off the track. the top and bottom support arms were the only thing holding the door on. an injury could occur,when the door drops to the ground if the door were to strike the child/person closing it. i have photos of the door. | |||

Without warning the top hinge of the hydraulic lift of the tailgate snapped while i was driving. hydraulic tube was sent into the ceiling of the van, causing the interior and ceiling to have severe damage. hatch cannot be opened without removing hydraulic assembly.*ak | |||

While opening the tailgate on the vehicle, my daughter was preparing to load merchandise into the rear . while she was under the opening tailgate a support pivot sheared off at the body. the tailgate came down on her head. the support went to a fully extended position. it lodged between the rear interior panel and the rear body work. once my daughter was out from under the tailgate, the tailgate closed. but because the support was fully extended it continued up through the interior of the van,put a hole in the interior roof panel and the end of the support abraided wiring above the roof panel causing the fuse for the air bags and the rear window wiper/washer to blow. as a result the dash mounted air bag warning light came on and of course the rear wiper was inop. i took the van into the dealer, because i read on your website of a recall pertaining to the liftgate supports. he informed me that my van was fixed once under the recall notice and there was nothing he could do for me. (he didn't even give me the time of day to look at what had happened!) i requested the phone # for chrysler consumer services, which he gave me. chrysler told me to re-contact the dealership and that if upon inspection the dealer finds that the broken pivot is covered by the recall (#730) they would fix it. otherwise i.m liable for the repairs. to date , i have rescheduled the appointment with the dealer. | |||
