Defect SummaryVehicle description: transit buses equipped with vanner 60 series battery equalizers. the equalizer could fail to operate properly and cause malfunction of the battery charging system.
Consequence SummaryWithout fusing, the equalizer may melt, burn, or smoke, possibly resulting in a fire or a smoke filled bus, thus necessitating a passenger evacuation of the bus. the failure mode can cause operator distraction, and/or possible personal injury.
Corrective SummaryPrevost will notify owners to inspect their buses for certain equalizers by the date code. the affected equalizers will be replaced. owner notification began november 29, 2000. owners who take their vehicles to an authorized dealer on an agreed upon service date and do not receive the free remedy within a reasonable time should contact prevost at 1-418-831-2046 or vanner at 1-800-227-6937.

Defect SummaryOn certain coaches and bus shells equipped with electronic throttle control/accelerator pedals supplied by williams controls industries, the weld attaching the clevis to the plate has insufficient penetration and could allow the clevis to separate from the plate.
Consequence SummaryIf the clevis separated from the plate while the vehicle was in use, the vehicle would return to idle, increasing the risk of a crash.
Corrective SummaryDealers will replace these pedals. owner notification began december 20, 2001. owners who take their vehicles to an authorized dealer on an agreed upon service date and do not receive the free remedy within a reasonable time should contact prevost at 1-418-831-5432.

 Date and tiem of event: oct 31.2010 time approx 1700 hrs cdt vehicle involved 2001 45 prevost xl ii bus prevost liberty converted motorhome vehicel has been owned by me for approx 16 months vehicle is kept in a fully enclosed garage and all tires are parked on painted plyboard dot date on bad tire 1506 michelin model x x pilote xza1 other numbers found on the tire were 61433 008olu 30283b tire showed no shape or forms of deficts. tire was remotely monitored by a tire pressure senso with monitors pressure and temperature. i found the inside dual on the passenger side flat. expecting a leaking valve stem extension i replaced sich with a new one and started airing up the tire. i have onlay a small sears 150 lb pancake compressor so this was a slow process. liberty recomends running 95 lbs on the drive axle. as i neared that prressure i stopped regularly and checked pressure for accuracy. at approx 93-95 lbs the tire exploded in my face. what i think saved my life was the outside dual. i suffered no eye trauma but have suffered ear trauma and post traumatic stress sundrone. i have been to the emergency room twice and now going to a ear noe and throat specialist for the ear problems. i have made numerous visits to the dr since for regular examinatins from this incident. upon the explosion the local fire dept and sheriffs office did respond to our home wherer the accident occured. i can attach such reports if needed. i deem this a very serious life threatening situation that is completely avoidable. any help in this matter would be greatly appreciated and i dont want to see anyone go thru this type of avoidable trauma. thanks bill price
