Defect SummaryGm is recalling certain model year 2006-2009 buick, lucerne; cadillac dts; hummer h2; model year 2008-2009 buick enclave; cadillac cts; model year 2007-2009 cadillac escalade, escalade esv, escalade ext; chevrolet avalanche, silverado, suburban, tahoe; gmc acadia, sierra, yukon, yukon xl; saturn outlook; and model year 2009 chevrolet traverse vehicles equipped with a heated washer fluid system (hwfs). a recall was implemented in 2008 to add a fuse to the control circuit harness to address the potential consequences of a printed circuit board (pcb) electrical short. however, there have been new reports of thermal incidents on hwfs modules after this improvement was installed. these incidents resulted from a new failure mode attributed to the device's thermal protection feature. the significance varies from minor distortion to considerable melting of the plastic around the hwfs fluid chamber.
Consequence SummaryIt is possible for the heated washer module to ignite and a fire may occur.
Corrective SummaryDealers will permanently disable and remove the heated washer fluid module. an updated page for the owner manual will be provided and inserted in the owner manual to document that the feature has been permanently disabled and removed from the vehicle. the safety recall is expected to begin on or before june 11, 2010. owners may contact buick at 1-866-608-8080, cadillac at 1-866-982-2339, chevrolet at 1-800-630-2438, saturn at 1-800-972-8876, gmc at 1-866-996-9463 and hummer at 1-800-732-5493 or at the owner center at www.gmownercenter.com.

My driver side headlight (low beam) was working intermittently. finally it stopped working. i bought a new bulb and was replacing it this weekend. while removing the old bulb, i saw that one wire had pulled out of the socket. i looked closer at the socket and realized that it was melted. there were also burn marks on the bulb plug and a green plastic piece. to my knowledge, this is an oem part. the part says phillips h11 ll and made in germany. it also says 55 w, 12 v. this would imply almost 5 a of current, which seems alot for the very small wiring size. while trying to find a replacement socket, i found out that this seems to be a very common problem on these vehicles. *tr

The low beam headlight went out on the vehicle. i went to autozone to purchase the headlight replacement when i was told that it could possibly be two different headlights that i would need to bring the broken one so they can sell me a replacement. i found out that it is not very easy to replace the headlight on this vehicle as several other parts have to be removed in order to get to the headlight connector. i found instructions and several complaints from owners indicating they have replaced the headlight several times during the year only to be told by dealers that the harness/connector are burned and that is the cause of the problem. i proceeded to remove the underliner of the tire to get to the headlight on my vehicle. as suspected, the connector and wire harness were burned. i found several service bulletins instructing dealers to check and replace the harness and headlight on these vehicles. when i called saturn i was informed that the dealership had replaced the headlight in my vehicle on april 2011...however they never informed me of this change. right now my wife is driving this vehicle without a headlight as saturn is refusing to replace the burned harness that i found on the car. this is not only a safety issue, but a fire hazard on the vehicle. this should be a mandatory recall by now....it is not just happening on saturn outlooks, it is also happening on gmc acadias which is exactly the same car..... *tr

Water leak from sunroof traveled down the driver's pillar and through the dash, onto the floor. this caused an electrical shortage which caused the emergency brake warning bell to sound. water was also leaking out of the curtain airbag badge on the pillar potentially compromising the airbag. water soaked the driver's carpet and mats. now the dashboard indicator light for rpm is operating sporadically.

My 2008 saturn outlook (w/sunroof) engine continues to run with key off position (see gm buletin #08-08-57-003c [seam seal] dated july 15, 2011) water leak at a-pillar caused various electrical problems such as engine continues to run with key off/possible no crank/no start/communication loss/various electrical concerns, which was the result of a water leak on the ip bec from the sunroof drain hose that exits through the vehicle at the plenum and re-enter through the unsealed seam at the front of the dash. this can effect 2007-2010 outlook; 2007-2012 gmc acadia; 2008-2012 buick enclave. gm only covers this to 3 yrs/36k mile warranty then gm requires owner to pay $400 to repair their engineering defect...

The passenager headlight burned out and was replaced. two weeks later the same light burned out. this time all the wiring was pulled out and it was discovered that the wiring harness was melted. looking on online it has been showned to be a big problem. as i am driving my 3 children around, i would like my headlights to work and not be melting in my car. please have gmc fix this problem. a car that cost as much as mine should have working lights and non-melting parts.

The headlamp on my 2008 saturn ourlook has gone out multiple times over the last two years that i have owned the vehicle. i have had to have the wiring harness replaced more than 3 times. the first time the new harness lasted a couple of months. the last two times, it only lasted about a week before burning up again. the harness that holds the lamp gets too hot and melts which is an electrical safety issue.

My 2008 saturn outlook lost both headlights in december 2010 within a couple of days of each other. the dealer informed us the bulbs had burned out and when they burned out they burned/corroded the wiring harnesses connected to them, necessitating a $200 repair. less than a year later, in november 2011, the right headlight went out again. when we took it to the dealer on november 4, 2011, they informed us the bulb had burned out and the harness had burned out again, and said the repair would be more than $200. we have seen this problem documented by many saturn owners on saturn fan sites, and also read to contact nhtsa to notify you of this common and potentially dangerous safety problem with the saturn outlook so that you could consider ordering gm to recall these vehicles. i hope you will order a recall and order gm to solve an unnecessary design problem that is a nuisance at best and could potentially lead to a fire or lost visibility that could result in accidents at worst. thank you for your consideration.

Passenger side headlamp wiring harness found melted during hid lamp replacement. this is a fire hazard. it appears the melted wire was due to a designmanufacturing defect in that the wiring going to the hid headlamp needs to be made from a larger gauge. also, there is not enough slack in the bulb assembly to easily replace the bulb.

The headlights went out while i was driving home in the early evening, reducing my visability and the ability for others to see my vehicle. fortunately, it wasn't night yet and i was still able to navigate home, however had it been just an hour later it would have been completely dark and siginificantly more unsafe for this incident to have occurred. the wiring harness is a known defect by gm and steps should be taken to remedy it. this can create unsafe road conditions for saturn outlook drivers.

Driver side headlight went out, bought a new bulb and went to change it and realized the wiring harness is melted. called the authorized saturn dealer and was told this was fairly common (what!!!!) and that it would cost approx. $700.00 to repair. after some on-line research, found alot of people out there with the same problem. factory warranty that would cover this ran out 4 months prior to this and do not have that kind of money to spend on a simple headlight. this should be a safety issue due to the fire hazard of melting wires and people driving with only 1 or no headlights. who knows how long i have till the passenger side does the same thing.

My saturn outlook only has 44,000 miles on it and the passenger side headlight went out. got a replacement bulb from dealership - changed it - and the light was out after a week and a half. went back to dealership to buy another bulb and when switching it out - noticed that the wiring harness was melted. looked online only to find that this is a common problem with this vehicle and it should be recalled - as the danger of a fire is a high risk and having working headlights is the law.

In march, 2011, my cars left headlight burned out. when i changed the light, i discovered that the wiring harness had melted. i replaced the harness with a gm part bought at a dealership. last week, the right headlight burned out. again, when i changed the light, i discovered that the wiring harness also had melted. the wires seem small to me, and are smaller than the wires on the replacement harness.

Been having intermittent problems with the drivers side low beam not working, have been on a few occassions to make it work cycling light switch and now that does not help either. was researching ways to attempt to repair and my sister inlaw also has a saturn outlook and was telling me hers ended up being the harness, and after seeing all the threads about this happening to alot of other saturns i think i will be taking my wifes car to a local dealership to be look at for sfatey ressons.

Low beam driver's side headlamp on saturn outlook. bulb appeared to fail but on further inspection of the wiring harness it was melted at the connector. this happened in jan/feb of 2011 and now (jul) the passenger side is out. replaced the light bulb initially on both only to have it continuously go out. inspecting wiring harness on the passenger side later today. this clearly appears to be a fire hazzard.

Driving south on interstate 77 in ohio the low beam headlights went out. luckily it was in an area that had some lighting that gave me time to react due to there being heavy traffic behind me that may have not been able to stop causing a fairly large accident. having to drive home with my high beams on the remaining 60 miles to home and then inspecting the bulbs when i arrived home, i noticed that the harnesses were melted near the connectors. i inspected the bulbs and they are the original equipment bulbs and not high wattage replacements that may have caused a deviation from what the harnesses were intended for.

Electrical failure- the headlights operate intermittently and produces a safety issue. i was ticketed in march for no headlights then 3 days later it started working. now 2 days ago it went out again. prior to this, i informed the dealer that our radio and driver side passender window operated intermittently as well but they could not troubleshoot the problem at the time, as if worked well. it appears to be a consistent problem with gm cars and we need an intervention.

The failure that occured in my 2008 saturn outlook was a faulty driver's side wiring harness for the headlight, which resulted in the lowbeam headlight being inoperable. this occured on may 8th, 2011 and was continuous. i had the wiring harness replaced at my cost due to the safety issue of only driving with one working headlight at night as well as the fact it's against the law in the state of ohio. additionally, i was traveling to canada and did not want to risk the possibility of a crash or collision with an animal due to the faulty light. i paid out of pocket over $270 for the necessary repairs including parts and labor (the old part is not available). gm published a bulletin regarding this issue: (#09-08-42-004: low beam headlamp bulb inoperative - replace harness and fill connector cavity for low beam bulb connector with nyogel grease may 29, 2009) this is most definitely a safety issue and should be recalled immediately. thank you for your time and consideration with this matter.

I have a 2008 saturn outlook xr. there are now over 150 complaints filed with the nhtsa regarding electrical problems and failure of the low beam lights on both sides. my passenger side failed and when repairs were started the gm repair shop noted the wiring harness where the bulb is inserted was burned (melted and blackened) as is the case in all the complaints i reviewed. gm issued a tsb in may 2009 addressing this problem (tsb# 09-08-42-004) which included filling the connector cavity with nyogel grease after removing and replacing the harness. my vehicle is out of warranty with just over 41,000 miles. the gm shop acknowledged that they have seen other vehicles in this model line with the same failure. however they had no directions to cover the repairs since the vehicle was out of warranty. it was recommended that i call gm customer assistance. that call resulted in an apparent lack of knowledge of this problem in spite of the numerous nhtsa complaints and internet discussions regarding this problem. this is an obvious safety issue should a failure occur while driving at night. i can also expect that the drivers side will also fail which will result in an additional repair cost beyond just replacing a burned out bulb.

Failure is headlights go out while driving vehicle. there is no indication of the headlights going out they just go out completely. both low-beam headlights on my 2008 saturn outlook went completely out while driving on the highway. luckily no accidents we're caused by this issue. i researched various websites only to find gm has a tsb on this issue tsb# 09-08-42-004. i contacted gm today to advise i would like to have this issue repaired they informed me this is only covered under warranty. this seems to be a very serious issue to only be covered under warranty and not under a safety recall.

Upon noticing a headlight out on my wife's 2008 saturn outlook, it was noted upon disassembly of the headlight that there was significant melting damage to the electrical wiring harness to the passenger side hid headlight fixture. the wiring harness was charred and blackened in the area of the headlight connector assembly. it appears there was signficant heat/arcing during the failure event. the damaged wiring harness caused the passenger side headlight to become non functional. the vehicle has approximately 50000 miles on it. this is the first such failure for this vehicle, however, upon reading various online postings about this matter, it appears to be a very common failure mode in these model vehicles. the headlight wiring harness assembly has been replaced, along with the headlight assembly due to heat damage to the headlight lamp wiring connector. old parts are not available due to being thrown away.

Overheating of low beam headlamp wiring causing melting of plastic components. headlamps will loose electrical connection and all lamp light will be lost.
11/10/201041000
Passenger side headlight became inoperative, wire harness replaced due to repair description of melting. no incident for approx 9 months. driver side headlight becomes inoperative intermittently during driving. hand adjustment of wire harness temporarily allows headlight to function. passenger side headlight begins to fail in same intermittent way as driver side headlight. loss of headlight while driving, driving becomes hazardous.

2008 saturn outlook wiring harness on headlight overheated and headlight no longer works. already had at service center twice. paid for replacement headlight which went out a day or two later; then paid for a diagnosis. already pulled over twice by law enforcement for having headlight out.

Passenger side headlight out on the 2008 saturn outlook. went out one day, a few days later started working again. now is out for good on low beam will work for high beams.

Our 2008 saturn just had to have the driver's side low lamp headlight fixed. melted harness piece. we had to pay for this one because we are out of warranty time (60k miles). the passenger side lamp went out while under warranty. it's ridiculous that we just spent $279 to have a light repaired that seems to be a recurring problem with saturns... before that we had the whole passenger side airbag light problem. the car doesn't sense when someone is sitting in the front passenger seat. dealership said car is out of warranty and it will cost us because it's probably a faulty sensor. this is our second outlook. lemon-lawed the 2007 and thought we'd give them another chance. not smart on our part in retrospect!

Driver's side headlight went out. replaced bulb. approximately 3 months later, same headlight went out again. replaced bulb. same headlight went out again in july 2010. replaced a 3rd time. bulb went out again in nov 2010. informed wiring had melted and harness/connector needs to be replaced. yesterday 12/21/2010 passenger side headlight went out and same problem.

The passenger headlamp stopprd working. we found out that this is because the wire and socket melted and burned. i'm relieved that it has not caused a major vehicle fire. 40,000 miles.

My passanger headlight turned off around 11/12/10, the bulb was replaced. two weeks later the same headlight turned off again. i took it back to the chevrolett dealer and they want me to pay $285 to fix the problem. i dont think that its right to make us pay for something that is completely faulty. this is really serious and can cause and accident.

Drivers side light bulb burned out. after investigation, the wire bundle to the light overheated/burnt and melted, resulting in the halogen bulb failure. unbelievable amount of online complaints involving the same issue, which repeats itself, even after the dealer replaces the wiring harness. the wiring harness is plastic, under-engineered and repeatedly fails. this item needs to be recalled and repaced with a metal wiring harness. reference saturn' s tsb #09-08-42-004. this is a known problem and a serious safety issue.

One evening while driving my children home from an event, we smelled a toxic burning type smell. when we arrived home, i saw that the front headlamp was out (a bit surprising for a 2 year old vehicle). the dealer discovered that the wiring harness connector was burned. a bit frightening and not tro mention costly. although the car was 2 years old, it has just above 36,000 miles and thus out of warranty. my fault for not getting to the dealer immediately when the mileage would have been just below the 36,000 mile mark.

 1. passenger side low beam went out. 2. replaced it thinking the bulb was dead. 3. 2 days later, the new bulb was out. 4. went digging back under the wheel well and through some funky arm bending and twisting of the neck i could see that one of the female lugs on the connector was burnt. 5. used electrical tape to temp fix the light. it went out 1 day later and not even driving the vehicle for that 2 day period. 6. went in and started an internet search and discovered i am not the only one with this major gm problem. i have already got a warning for the police to fix the headlight out. i shouldn't have to worry when it will go out again and hopefully not get a ticket or even worse. a crash.

The wiring harness on the passengers side low beam has melted twice. it was last replaced two weeks ago, and has since gone out again, i suspect that it has melted again but will not know for sure until i take the car back to the shop. why rae these harnes's melting and why hasn't gm made a recommendation to fix this problem.

2008 outlook had a burned out low beam on the driver side at 31,750 miles. dealer confirmed that the wiring harness was melted causing the failure and need replacing. gm needs to recall both wiring harnesses as i am sure the next one will go and it will be past the warranty. this is clearly a safety defect having a light's wiring harness melt. apparently, this is a common problem with '08 outlooks. please issue a recall!

My 2008 saturn outlook has had problems with the headlights. i had two headlights malfunction at the same time. i went to replace the bulbs and one of lights started to work without replacement. the second bulb had a burn mark and the connector on the wiring harness was melted. i plugged the bulb back it and it work, though a bit dimmer. this is the 3rd or 4th time i've had problems with the headlights. i had thought they replaced the wiring harness before under warranty. i'm assuming they will be doing the same since the bulbs still work and the connector is melted on the one harness.

The right front headlight went out, replaced the bulb and realized the connector was melted. two weeks later the left front bulb went out, brought it to the dealership and the connecter was melted. so both sides had electrical melting. cost to repair is $352.

I have a 2008 saturn outlook and the right low beam went out and i found that the connector had melted. the very next day in the evening the left low beam went out and i was without low beam lights. i had both wireing harnesses replaced at a cost of almost $400 on 8/27/10 i hope this fixes my problem. major safety issue in my opinion. i had to drive with high beams and that was not good for other drivers and they were angry with me. needs recall.

I own a 2008 saturn outlook, which i purchased new in august of 2007. in february of 2010, i noticed my right low-beam headlight/daytime running light was out. i contacted my local saturn service center and was told that there had been some problems with the wiring harness and i should bring it in for a replacement harness, since it was still under warranty, which i did. in april 2010, the left low beam headlight/daytime running light went out. i again contacted saturn and asked if, assuming this was the same problem, it would be covered by warranty, since they had already acknowleged that a problem existed with this harness (the vehicle was about 2,000 miles over warranty by this time). i was told that, since there had been no recall, it would not be a warranty item, and that i would have to pay for the repairs myself. since the saturn dealer is about 50 miles away, and it was not going to be covered by warranty, i opted to have it repaired at a shop much closer to home. after determining that the bulb was functional, they ordered a new wiring harness, and installed it in less than an hour (to the tune 0f $171.44). i asked to see the old harness, and upon inspection, i observed that the housing around one of the connection points was clearly burnt and melted. i have heard of numerous other reports of this failure, much like the recalled wiring harness for the heated windhield wipers. this harness defect clearly is a safety issue and should be recalled.

Wire harness for front head lights defective. i was charged $25.00 to get the pass side fixed six weeks ago, now they want $200.00 to fix the driver side for the same problem.

We have a 2008 saturn outlook. the headlights keep going out. the wiring harness is bad and the expense to replace is very high ($750). this is a common problem and many people have submitted complaints and commented in forums on the web. the problem occurs at random times. during daylight, but more dangerously while driving at night on the highway. there is no warning. i have replaced 7 bulbs in the last 6 months and tonight both lights stopped working while driving home. in order to get home in safety i had to switch to high beam lights. there is an obvious flaw with this line of vehicles.

Saturn outlook xr headlights went out when wire on left front headlight melted wiring.. this happened in january 2010 when the right front went out and melted the wiring. cost to replace the one in january $640.00 and was told today to replace the left front will be $750.00. this is a defect and should be recalled as you can look at many web sites and many people are having this problem. we noticed that this only happens when the lights are on high beam only. *tr

I purchased this 2008 vehicle in july 2008, used but new with a bit over 16,000 miles on it from a delearship in syracuse that is no longer active. my vehicle has always been well cared for and serviced accordingly. starting jan. 31, 2010, the engine began to make a loud knock. it did not matter if i warmed the vehicle, or if it was warm weather or cold. on some instances it seemed to not accelerate on the highway as it should. often hesitating to go. it has been into the dealership four different occasions, for a total of 21 days out-of-service cumulatively, soon to be more. the first time they gave it back the next day as they stated they could not duplicate the noise. three days later the knock was back. the second time it went in they had for 7 days, could not duplicate. 2 days later back again...the third time the dealership locally here put a new timing chain in. they said it was slack. however, this did not fix the issue. it was supposed to go back in for the same engine knocking, etc. before it could go in i was driving at night with small children in the car on a highway and had no headlights. after taking it to the dealership they say the wiring harness melted and they have no idea why. they take the vehicle in again for a fourth time. tell me its fixed. new wiring harness. pick up vehicle..same engine knock present. they now say maybe its a lifter? cam? don't have parts. give me this same vehicle to continue to drive. something is fishy here. how many vehicles does it take for a safety recall or investigation????? does someone have to die?! headlights going out at night on highway, engine not accelerating....major safety issues. multiple complaints. i'd rather not have someone called to the scene of my death or the deaths of my children because of a known defect that is not being addressed! also, another 2008 saturn outlook is at the same dealership for similiar problems...
