Defect SummaryToyota is recalling certain model year 2004-2010 passenger vehicles. the accelerator pedal can get stuck in the wide open position due to its being trapped by an unsecured or incompatible driver's floor mat.
Consequence SummaryA stuck open accelerator pedal may result in very high vehicle speeds and make it difficult to stop the vehicle, which could cause a crash, serious injury or death.
Corrective SummaryToyota filed an amended defect report on november 25, 2009, stating that dealers will modify the accelerator pedal and, on certain vehicles, alter the shape of the floor surface under the pedal. these changes address the risk of pedal entrapment due to interference with the floor mat. redesigned accelerator pedals will become available beginning in april 2010 and dealers will replace any modified pedal with the new pedal if desired. also, dealers will replace any genuine toyota or lexus all-weather floor mats with redesigned all-weather mats, or repurchase the previous mats from owners who do not want the new ones. additionally, software modifications will be installed on camry, avalon and lexus es 350, is 350 and is 250 models that will ensure that the brake overrides the accelerator in the event both brake and accelerator pedals are applied. toyota will begin mailing letters to owners in december 2009. owners may contact toyota at 1-800-331-4331, lexus at 1-800-255-3987.

 On 8/29/12, i got into my 2008 prius in the parking lot of the easton, md giant food-store to return home. this lot is on a slope just above a parking lot for walmart, with a paved road between the 2 lots. the parking lots were very crowded. i needed to get to the road separating the 2 lots in order to go home. i had just started the prius and put it into drive when it started to roar loudly, jumped out fast, and accelerated rapidly, even though i brake multiple times. the brakes seemed to make it accelerate faster. i was not touching the gas pedal, only the brake, but the car was roaring and out of control. on the way out of the giant lot, the prius hit 3 cars. the noise was very loud, and the car was out of control. pumping the brakes or holding them down did nothing to slow the car, and the noise was terrifying. the brakes did not work, and the prius continued to make a very loud roaring noise. the speed of the car made steering difficult, if not impossible, and there could have been injuries. the prius then left the giant parking lot, went down the mulched slope, crossed the street, bumped over the curb, and went up the walmart slope hitting a small tree. the car was probably going at least 40 mph and came to a stop only after it plunged into an unoccupied black car parked in the walmart parking lot. it was severely damaged and towed away. no one knew who the owner was at the time the police ordered it towed away. an occupant would have been severely injured or killed.during the above, the prius continued to run and the airbag functioned, keeping me uninjured. the brakes and the accelerator failed totally. four vehicles were damaged, one of which was totaled. but as far as i know, there were no injuries??for which i am grateful. this prius should not have been on the road despite having only 74,000 miles and its apparent good condition. *tr

 Around 4:30pm on saturday afternoon, we opened the car door to run some errands and were startled by a piercing noise. the dashboard was lighting up (abs & vsc) and although the car started, the dashboard remained lit and the beeping continued. when driving the car to the dealership, the brakes did not brake fully. after a $58 diagnostic test, we were told that acc pressure sensor needed to be cleared and if we returned with the same problem, we may need to replace a brake actuator part (cost = $2,560). before this incident, on saturday afternoon, we drove the car on the interstate for about 150 miles. for the last 15 minutes of our trip, we were driving through heavy rains on the interstate and on side roads. we did not experience any problems when driving the car in the rain, but are wondering whether or not the water affected the brakes. *tr

 While entering a highway onramp in my company's 2008 toyota prius, the car suddenly began accelerating out of control without application of the gas pedal. i had to press hard on the brake just to hold the car at 65 mph. the car was surging and lunging forward, and i worried i wouldn't be able to find a place to pull over before losing control completely. for the next 5-10 minutes, as i approached traffic in front of me, the car continued to accelerate, even though i had my foot on the brake the entire time. at one point i noticed the car was reaching speeds in excess of 95 mph. a loud rattling noise ensued and an electrical smell filled the car. eventually, the brake completely failed to have any effect, and, barelling down the highway at 90 mph, i put the car in neutral, but it continued to accelerate. at this point i had my right foot on the brake pedal and depressed the parking break all the way to the floor with my left foot, this slowed the car slightly, but not enough to regain control. finally, i turned the car off completely, and with my hazard lights on, managed to pull the car away from traffic and off on the shoulder of the highway, where i coasted for a long distance, speeds decreasing gradually until eventually it stopped. smoke and a burning electrical smell filled the car. i filed a police report with an officer who happened to also be on the shoulder, giving someone a speeding ticket. my company towed the car to a dealership, and an investigation is supposedly underway, though i'm not optimistic about any sort of resolution based on the testimonies i have read from others who have experienced the same thing. luckily, in my case, this didn't result in an accident, and noone was killed.

 2008 toyota prius, when i hit a bump or pothole at relatively low speed, the car sometimes goes into anti-skid mode, leading to a brief disconcerting period of lurching/acceleration/loss of braking ability before the car returns to normal. this problem is usually accompanied by the yellow anti - skid icon indicator on the dash.

 The instrument panel on a 2008 prius decides not work at varied times. it is a sporadic problem which occurs then goes away. you could be driving down the highway at night then you see that you have no display telling you: 1) how fast you are going, 2) how much gas do you have, or 3) distance traveled. it's too difficult to troubleshoot because the problem would goes away.

 Rapid acceleration while cruise control engaged. while cruise control was set for 68 mph, i found it necessary to pass a semi. while the cruise was engaged the accelerator was depressed and i was able to pass the truck not having to go any faster than 73. shortly after, i became aware that the car was continuing to accelerate and i was unable disengage cruise control by braking once normally and braking once hard....the acceleration continued. speeds were in excess of 93+ and i was finally able to stop the acceleration by disengaging the cruise at the switch. braking should always disengage cruise and should have slowed the vehicle. it did neither. fortunately i was on an open freeway....with only weather issues....but this could have been very serious or fatal should traffic had been stopped in front of me.

 Tl-the contact owns a 2008 toyota prius. while driving approximately 5 mph into a parking space. the contact engaged the brake pedal and the engined revved extremely high with an increase in the acceleration. the vehicle would not stop and traveled over a cement parking block. the vehicle continued to accelerate and crashed into a brick wall. the air bags failed to deploy with the impact involved. there was no personal injury. a police report was filed of the incident. the estimated cost for the brick wall damages was $2,500. the vehicle was repaired under the nhtsa campaign id number 09v388000 (vehicle speed control: accelerator pedal) in june of 2010. the contact stated the remedy did not correct the problem. the vehicle was towed to a collision center. the failure mileage was approximately 30,000. js

 Tl- the contact owns 2008 toyota prius. while traveling 40 mph the driver tried to brake and the vehicle started to accelerate. the driver pulled into the grass on the side of the road trying to stop the vehicle because the brakes did not work. the driver traveled 400 yards on the side of the road and hit many bumps in the road before the vehicle came to a stop. the driver stated that the brake light came on and then the vehicle was able to be stopped. the brakes were smoking when the vehicle stopped. the front driver side tire blows out during the incident. the state police came and had the vehicle towed to a toyota dealer and the vehicle was still there at the time of the complaint. the vehicle had not been inspected at the time of the complaint. the failure mileage and current mileage was 22000.rl

 While driving about 65 mph on a smooth interstate, my 2008 toyota prius accelerated when i tapped the brake to take it out of cruise control. i stepped on the brake again and it stopped accelerating, and slowed down. i had removed the floor mat a few weeks prior. i took it to toyota to have the recall work done, which included a software upgrade. they said after they inspected the car that they determined form the log that the brake and accelerator had been pressed at the same time, twice. i am sure i did not step on both. as i said, i tapped the break to take it out of cruise control, and at the time it accelerated my foot was in the air, not touching either pedal. i suggested the cruise control may be erroneously putting flags in the log as it slows and accelerated to maintain a consistent speed. the dealer refused to report this suggestion or since it would be diagnosing, and said there was no place for them to report that i was sure i had not stepped on both pedals at the same time. thank you, and i hope you are able to resolve this problem. *tr

 I was trying to park my car(toyota prius 2008) in the farmers market and the car didnot stop and continued to accellarate and went further about 50 feet and hit a wood pole. later toyota service fixed the problem by changing the necessary parts in the vehicle. they confirmed that they took the car for test drive and made sure that everything is ok. but i am still scared to drive that car because of what happened. i have been driving for more than 18 years in this country and do not have any traffic violations or accidents. this was the first time anything like this happened. i also own two other vehicles , honda oddasy and toyota camery.

 Hi, while driving to work this morning, my left headlight flashed and then stopped working. my 2008 toyota prius is about 1.75 years old with approximately 28,000 miles. i have heard from other prius owners that they have had problems with their headlights. i have also heard the replacement cost for the headlight is $300.00. i find that hard to believe, nonetheless headlights in everyother car i have owned seem to have a life of 10 or more years. this is a safety issue because i drive to work early in the morning when it is dark and i sometimes don't return home until after dark. the bulk of my driving has been in the daylight, however i need my car to get to work. also on 3 occasions my cruise control accelerated beyond where i set it. for example i reset the control after having hit the brakes. i got back to my speed and set the cruise control at 70mph. i noticed a short while later the car was going 95. i hit the brakes to slow down and the car immediately came out of cruise control and slowed down as it should. i now take the car all the way out of cruise control shutting it off after i hit the brakes. i next turn the control back on and reset the cruise. using the cruise this way i have not had the problem of the car speeding up. ii figure i will continue to use the cruise this way and observe whether there is any problem. however,i don't believe toyota intended for the cruise control to be used this way. thank you *tr

 2008 toyota prius. consumer states unintended acceleration. *tgw while driving on the freeway, the consumer let up off the accelerator, but the vehicle continued to speed up. the consumer released her foot completely from the accelerator all together, but the vehicle increased in speed. the brake was applied, but it did not slow it down at first. finally, the brake began to work. the consumer was able to drive safely to the dealer. the dealer was unable to find anything wrong with the vehicle. the consumer also stated the floor mat was removed months ago. *jb

 Toyota prius 2008 less serious - frequent release or lapse of braking by abs (assumed) on irregular roadway small cracks seem to confuse system with significant forward travel of vehicle before brakes reengage. similar to my earlier 2005 model but more frequent occurrences and longer lapse in pad rotor contact during braking. most serious brakes failed almost 10 seconds. travel on wet slushy highway for 2 hours on 55 mph road with uphill grade and slight turn to right driver applied light brake pressure attempting to time deceleration to controlled intersection for green light. application of brakes slowed vehicle no more then uphill coast, firmer pressure to no effect, full hard pressure no effect, slight release and full slam, no immediate effect, finally abs stuttered/pulsed and stop vehicle. *tr

 When hitting small bumps travelling at speeds of 25-50mph (such as bumps before bridges) the abs will activate and the pedals go loose. it seems the car can not be controlled for about 3/4 of a second once this occurs. luckily i have never been rounding a turn or approaching stopped traffic when this occurs or i would not have been able to stop. i had one close call where the brakes seemed to lag before kicking in, then jolting hard after the delay causing me to almost lose control of the vehicle.

 My 2008 prius suddenly accelerated as i pulled over to park at the curb 2 hours ago. my foot was on the brake, not near the accelerator, and i long ago replaced my floor mats so i know there was no mechanical interference. this is the 2nd time i have had this experience with the car. circumstances were identical in the first occurrence. no damage resulted as the acceleration seemed to pulse for less than 1 second and i did not lose control of the vehicle. i did notice some gravel in the road as i approached the parking spot but i am confident that the vehicle did not skid.

 I was parking my car head in first, in an apartment parking lot, i had just pulled into the space, put my foot on the brake and depressed the park button, i then began to reach for the power button when the car uncontrollably lunged forward going up a slight incline, hitting a chain link fence pole and coming to a stop damaging the front bumper. i don't know if it was the brake, or electrical system but, on occasion since this incident, i have accidentally turned the cruise control on and felt a surge of speed but the car was slowed/stopped by using the brake. it's so easy to turn the cruise control on without knowing it and i wonder if it could be a contributing factor to some of the prius problems. *tr

 At 15k miles the car started to lurch when applying brake. called dealer and was simply told that is how the prius is. this action continued and every time it happened we called the dealer and got the same answer. at 60k we were on our way to service for regular maintenance and 2 blocks away the car lurched into traffic. that was the most aggressive the car had ever got and it scared us to the core. when we arrived at the dealer we told them about what just occured and the manager said he wanted to go on a test drive and we were happy to hear it. the manager sat in the car and then decided against the test drive. we were then told that they could research the problem but it would be at a cost to us unless we get the service done as well. we were bothered and couldn't understand why we were being treated this way. we left that dealer frustrated and angry. we then took it to another dealer whom inspected the car and said everything looked fine and by the way we show brake and gas being applied at the same time. the tech said you should stop driving with 2 feet. we left that ridiculousness and called toyota complaint dept. the call was in dec. 2009 and they didn't see out car until feb. 2010. and after inspecting the car was given the exact same diagnosis. we avoid driving the car as much as possible. we told the master tech that our dash board shut down while driving on the frwy at 65mph and has the new added problem of lurching while already in motion and we are afraid of our car and for our safety. the master tech said it was all operator error. that appears to be the end result with toyota and we are highly disappointed. we have been toyota customers since 1992 and now pay for a car we avoid driving whenever possible. we just want to be in a safe car on the road and feel secure not fear. *tr

 Toyota prius problem - cruise control malfunction: on 2 jan 2010 (about 0600 hrs) i was driving a rented prius (alamo) from palm beach, fl to miami, fl. i was using cruise control and going the listed speed limit for i-95 (65mph). the car seemed to be performing correctly for the first 35 plus miles but then i got into heavy traffic and had to brake. the cruise control released normally and i slowed to about 60 mph. when the traffic cleared i pushed the cruise control resume button and the prius started to accelerate as normal. however, the acceleration did not stop at the original speed i had set and had been driving at (65 mph) but continued to climb to 70 mph and higher to near 75 mph when i had to hit the brakes and shut the cruise control off to slow the car. after that i did not use the cruise control for the remainder of my trip. i just thought there may have been a glitch or operator error and i did not think of it as the problem i recently saw on the news. i live in the republic of panama and did not see anything about the prius problems until recently. as i said the car was a rental and i just mentioned to the check in person that i had a slight problem with the cruise control but i do not know if it was reported. *tr

 My mother has a 2008 toyota prius and was pulling into my driveway. the approach to the driveway is uphill and the driveway is more of a parking area with an access apron. for some unknown reason, the car's engine revved (very high rpms - i could hear it from the basement). the car grazed the side of the house, went through a short wall (brick - about 1 foot tall, 1 foot thick and 5 feet long) and an iron deck gate. the car ended up on the deck after breaking off two 6x6 support posts. the deck has horizontal stainless steel cables instead of pickets, and i believe that these are what kept the car from going over the edge - a 25 foot drop. *tr

 I wanted to include my voice in the recent complaints about the toyota prius brakes. i have a 2008 prius and have experienced the same issue several times- the brakes seem to stop working and the slick-road warning light comes on when i go over a big bump or pothole while braking. braking comes right back, but i can imagine this might be a bigger issue if there was a vehicle right in front of me, in inclement weather, etc. i have heard people only mention the 2010 prius model year, so i wanted to let someone know that the 2008 prius also seems to be affected... thank you. *tr

 2008 toyota prius - brakes slip when trying to stop at low speeds, especially on wet roads or on roads with bumps (including manhole covers). this has occurred repeatedly since i bought the vehicle. the dealer, on three separate occasions, said that they ran diagnostics and found no anomalies. i read today that this problem has also been reported on new (2009-2010) toyota prius models, and that the japanese government had ordered toyota to investigate. however the article said that toyota was unaware of any problems with older vehicles. i am filing this complaint in the hope that toyota will extend its investigation to series 2 prius models (2004-2008) as well. *tr

 We were stopped at a neighborhood intersection. as we began to let off of the brake to make a left hand turn, the car accelerated uncontrollably. depressing the brake had no effect on the acceleration. before we turned the wheel to the left to begin the turn, the car had sped across the 35 foot street, jumped a 6 1/2 inch curb, went through a fence, across another street, and was stopped when it hit a second fence. the total distance traveled from being stopped at the corner until the vehicle was stopped by the second fence was 81 feet. the streets were dry and there were no skid marks on the road. however, due to previous rains, the grass areas were moist. there are no tire marks in the grass from the curb until approximately 6 feet before the stopping point. the front lift of the prius is around 7 1/2 inches. the vehicle had to have significant momentum to go over a 6 1/2 inch curb in just 35 feet. we filed a claim with our insurance on jan. 7 and requested that the vehicle be inspected for accelerator issues. the car was towed to toyota whom, upon their own investigation, stated that there was no evidence of acceleration malfunction. the car is being repaired at a body shop with initial damage estimated at over $6000. we have photographs of the damage to the car as well as the scene of the accident and measurements of distances traveled. in 2009, we received a recall letter from toyota concerning floor mat issues with our prius. we inspected our factory-installed floor mats, as instructed by the recall letter, and were awaiting their 2nd notification when the campaign remedy became available. the floor mat on the driver's side has a locking mechanism and could have, in no way, obstructed the accelerator pedal. the car should be repaired and ready next week. however, we do not believe it is a safe vehicle to drive. what recommendations do you have? *tw *tr

 S10 appropriate handling - letter to the secretary from mrs. judy haley gizmo disputing claim by toyota that unintended acceleration of toyota vehicles due to faulty floor mats. *nj the consumer disputed toyota's claim that the many cases of unintended acceleration were due to faulty floor mats. the consumer stated the crash she had was not due to faulty floor mat's which were latched down, but was due to acceleration/brake problems in the computer and electrical systems of the vehicle. the consumer stated she backed the vehicle out of the driveway, she exited the garage and pressed the button to close the garage door. after closing the garage door, she sated she wasn't sure if she placed the gear in drive or neutral, but suddenly the vehicle lunged forward going up to 15-20 mph. within 5 seconds the consumer crashed into the garage door, rammed into the inside kitchen wall causing extensive damage. the air bags didn't deploy. the consumer suffered injuries.

 Accelerator pedal on vehicle failed to return to non accelerating position after accelerating on freeway. vehicle continued to accelerate even while braking. i had to wedge my foot underneath the accelerator pedal to lift it to the full upward position. this incident occurred while accelerating sharply, and has not occurred with gentle acceleration. i have avoided any sudden acceleration with this vehicle , for fear of a recurrence of this problem. there was no floor mat interference while this was occurring, it felt as though the accelerator pedal was being operated without foot contact, as i could feel the pedal depress underneath my resting foot. i am a very experienced driver and have driven commercially for 20 years and have never experienced anything like this. *tr

 Needing maximum acceleration to avoid being struck from behind, i thrust the accelerator to the floor in my '08 prius. safely avoiding a possible collision, my auto continued to accelerate. the accelerator was stuck in the full down position and could not be budged either by hand or by repeated stomping to release it. i made sure the floor mat was not interfering with the accelerator, which it wasn't. i braked hard and turned off the 4 lane street to the right and was about to turn off the engine when it released as rapidly as it began. i reported it to toyota at my next oil service - but they had no prior experience of this type of complaint. i have since refrained maximum acceleration - but am afraid to let anyone else drive my auto. now that i am aware that there is no direct mechanical connection to to investigate or correct in the fuel system, but instead an electronic one, (fly by wire like the new commercial jets), i am even more on edge. thank god, the prius is underpowered. in a more powerful auto, the engine would have overpowered the brakes and led to a possible disaster. electronics malfunction sporadically! please investigate!!!*tr

 I own a 2008 toyota prius. in april of 2008, 4 months after i bought the vehicle, i was pulling into a spot in the parking structure at work. all of a sudden the engine gunned and the car lurched forward, went over the bumper guard in the spot, and was stopped by the concrete overhang, which totally crunched the hood. at the time, i said to myself, why aren't the brakes working? i think this problem may be related to the gas pedal problems currently being reported concerning the toyotas. thanks for your assistance. *tr

 I am not being melodramatic when i say that i thought i was going to die today. my 2008 prius accelerated almost out of control. i was merging onto an expressway when the accelerator seemed to have a life of its own and took off at an incredibly high rate of speed. it would not stop, kept getting faster and faster. i managed to slow down the vehicle. it was fine for the 20 minute drive down that expressway. after getting off and getting into city traffic i was stopped at a red light. when the car lunged forward. thank god i had my foot hard on the brake. it did not brake away only lunged 6 - 12 inches. *tr

 I own a 2008 toyota prius. on 12jul2008 i was returning home and driving on hwy 70 east,near goldsboro, nc. i was traveling at a speed of approx 55-60 mph. the vehicle was in cruise control. the traffic was getting heavy and i had to slow down. so i put on the brakes and the cruise control did not respond or disengage at that time. so i kept applying pressure on the brake pedal and pulled off of the roadway. when i lifted my foot off of the brake pedal the vehicle again wanted to accelerate. i had to turn the ignition system off and wait a short time. i re-started the vehicle and every seem to be ok. on 13jul09 i had the vehicle checked out by a toyota of new bern, and they couldn't find anything wrong with the vehicle. they stated they would open a case on it and notify there company. i just wanted to write this notice in case it happens again to me or someone else. *tr

 2008 toyota prius. consumer states unintended acceleration *tgw the consumer stated as she was coming to a complete stop with her foot on the brake, the vehicle suddenly accelerated. within about 1/10th of a second, it was moving at a very high rate of speed. the vehicle did not increase speed gradually, but was almost instantly moving very fast. the consumer stated she could hear the engine revving quite loudly. the vehicle crossed a grassy area about 20 feet wide and then hit a post which was about 6 inches in diameter. it demolished the post and then plowed through some small trees and large shrubbery. the vehicle finally came to rest about 10 inches from the corner of a house. the consumer stated prior to the accident, she received a recall letter in the mail, and complied with the recall. however, since the recall was performed, she noticed that when the cruise control was activated and going up a rise in the road, the engine would start to audibly rev and the vehicle would gain 2, 3,4 mph over what was set for the cruise control speed. it also gained a few miles when on a downward slope. as soon as the road leveled out, it would go back to what the cruise control setting was. the consumer took the vehicle back to the dealer, but they dismissed it as not important and said it was a normal occurrence and it had nothing to do with the recall work they performed. *jb

 Ltr fwd on behalf of (mi) re toyota braking problem in her 2008 toyota prius, reply to lansing, mi ofc. *tgw the consumer was involved in accident. the consumer was stopped at a stop sign behind two vehicles going south at an intersection, waiting to turn right. the consumer stated her first practice in driving the vehicle, when starting from a stop is to simply take her foot off the brake pedal and allow it to start moving without putting her foot on the gas pedal, to maximize fuel efficiency. as she moved her foot toward the gas pedal, but not quite on the pedal, the vehicle suddenly lurched at a very high rpm and hit the vehicle in front of her, which was about 3 feet. she applied the brakes again, and the same thing happened. the consumer stated the damage to both vehicles were minimal. the consumer stated the incident happened again, but without incident. *jb
