 This incident was reported by family of the driver. vehicle had been modified for use by a disabled person. steering system was controlled by an electronic mobility system (model ds2000). while driving approximately 30 to 35 mph, driver lost all directional control of the vehicle, and was involved in a head-on collision when his vehicle wandered into oncoming traffic. the driver was killed . please provide any additional information / documentation / photographs.*ak

 Driver's side seatbelt retractor is stuck and will not come apart. also, transmission failed without prior warning. this was the sixth transmission.*ak after transmission was replaced, the engine began to stall. gas gauge failed. dashboard light shortened out. speedometer failed and had to be replaced 4 times. transmission not shifting properly. front end vibrates especially when turning. brakes are grinding. rear view mirror fell off. high mount brake light shortened out. side window and door rattling. rear wiper failed. muffler needs to be replaced. exhaust smell in van. engine overheating. air conditioner failed. vehicle pulls. consumer is requesting reimbursement. *tt

 My wife, terrie was driving eb on a local interstate at 65 mph when the spare tire fell onto the roadway from the bottom of the van. the spare was attached to the bottom rear of the van. she did not hit any bumps or debri on the roadway. upon examination of the device that held the spare, it was revealed that the cable that held the spare, was frayed and broken. luckily no one was traveling immediately behind her, or the tire may have crashed into their vehicle or oncoming traffic on the opposite side of the interstate. *ak

 Our van started showing holes in the paint in 1993 (very small on the hood). we tried to fix the problems with the touch up paint provided. this did not work. the paint started bubbling up and popping. each time it did that a hole would appear that got bigger and bigger. we later noticed the roof doing the same thing. every time we washed the vehicle (gently) more paint came off. every time we brought the car in for servicing (at dealers in richardson, tx and plano tx dealers we mentioned to the service managers the problem and asked if any other vehicles were having the same paint problems they indicated they had never heard nor seen any ofher vehicles with the same problems. they always indicated it was our problem. we asked how much it would cost to re-paint the van and were told that it would cost $2,000 or so dollars. they always indicated that it was our problem and no one else had had the problem. i have come to find out that this is a common problem with vehicles between 89-94 and that the paint/primer bond was the problem not potential customer neglect. our van statred rusting so we went to several paint places to get estimates for re-painting. we are now having the van re-painted for approx. $800. (not $2,000 as the dealer would have charged.) this should not be a customer problem. the paint peels off, goes into sewers/ground and pollutes. the rust weakens the structures of the vehicles and makes them less safe. fyi i notice loads of vehicles in the same years with the same problems. the jack stand bent when we were changing a time. the bottom just bent to the side. luckly no one was hurt as the tire was still attached when it bent.
