Defect SummaryMotor coach industries (mci) is recalling certain model year 2003, and 2007-2012 e and j models along with model year 2007-2012 d4000, d4005, d4500, d4505, d4000istv motor coaches equipped with meritor wabco electronic stability control (esc) modules, manufactured from october 2002, through october 2011. under certain road and driving conditions, vehicle body roll and road inclination characteristics may adversely affect the slip angle calculation of the electronic stability control (esc) system. this might cause the esc to perceive an over steering situation and therefore apply the outer wheel brake on the front axle until the vehicle is perceived to be stable.
Consequence SummaryIf the driver is slow to react during this esc intervention, the vehicle may deviate from the intended line of travel, increasing the risk of a crash.

Defect SummaryMci is recalling certain model year 2000-2009 d4000, model year 2001-2010 d4500 and model year 2005-2007 d4505 coaches equipped with ricon wheelchair lift (wcl) and a cassette-type wcl compartment door. the wheelchair lift may become disengaged from a locked position and slide unrestrained on its rollers in the storage structure, potentially breaking through the cassette door and extending beyond the outside of the door when the coach is in motion, particularly when the coach is making a left turn.

Consequence SummaryThe purpose of this standard is to minimize the likelihood of occupants being thrown from the bus and to provide a means of readily accessible emergency egress.

 This investigation was prompted by a review of service bulletins provided bymci to the agency. motor coach industries (mci) submitted a service bulletin/product improvement campaign (#3016) in their 579.5 monthly submission regarding an alternator cable chafing issue on d series coaches equipped with a caterpillar c13 engine and a 50dn delco alternator. the bulletin reported the cable can chafe against an engine bracket or as a result of movement within the cable clamps.the corrective action is to install a new cable and support brackets.odi believes that there is a high risk for a thermal event resulting from alternator cables chafing within the engine compartment.odi places increased emphasis on preventing potential fires involving motor coaches and other vehicles which carry large numbers of passengers.in november2009, mci filed a safety recall (09v-346) outlining theirremedy program to resolve this issue. mci will install a new cable with a heavy duty terminal that does not have a spiral loom over the cable passing through the cable clamps.also, the new clamp and mounting bracket were changed to better align the cable as it passes through the cable clamp thus preventing a chafing condition. the action taken by mci resolves this investigation.accordingly, this investigation is closed.

 Representatives from a large bus fleet operator have alleged that they have experienced several driveshaft failures on mci d series buses beginning in march 2010.the failed driveshafts were not contained by the safety loops for the driveshaft.they assert that in two instances the driveshaft failures resulted in loss of control of the bus and in multiple injuries and fatalities.the vehicles have a steerable tag axle that is used to increase the gvwr while reducing the turning radius and minimizing tire scrub that would normally occur with a tandem rear axle configuration.in these buses, the transmission couples to the driveshaft which then passes through a guardian structure integral with the tag axle and a forward safety loop before it couples to the drive axle.the driveshafts in these models with certain engine/transmission combinations may not be encapsulated by a safety loop (or guardian structure of the tag axle) at the transmission end.testing done by the fleet operator suggests that in the event of a driveshaft/transmission coupling failure the shorter driveshafts could escape the tag axle pass-through guardian structure and disable the steering lock of the tag axle and potentially damage other safety systems such as brake lines.additional vehicle dynamic testing suggests that an unlocked passive steerable tag axle could cause a loss of vehicle control given sufficient speed and other conditions.odi has reviewed the material supplied by the fleet operator and believes that additional information is required to determine if a safety related defect exists.odi is opening this investigation in order to gather information to assess whether the subject vehicles contain a safety related defect.
